Open Rhinoplasty in Children

Abstract: Septorhinoplasty in children carries the risk of growth disturbance of the nose and premaxilla. The open or external approach has the advantage that the cartilaginous nasal skeleton remains intact. This open approach enables the surgeon to excise dermoid cysts and to realign lower lateral cartilages (unilateral cleft) without disturbing the integrity of the cartilaginous skeleton. In our experience, absolute indications for open rhinoplasty in children include dermoid cyst, cleft lip nose, and septal abscess. … Show more

“…In dealing with septal abscesses in children and adults, some authors have gone so far as to recommend drainage via an external rhinoplasty approach followed by immediate reconstruction of the septum with autogenous conchal cartilage sutured to polydioxanane (PDS) foil (Ethicon, Somerville, NJ). 23 The diagnosis of a nasal fracture can usually be made by history and physical examination alone, thereby precluding the need for routine diagnostic imaging studies. However, in some cases where examination is limited, plain films in a lateral nasal projection and Water's view at 30 to 45 degrees can help identify bony fractures.…”

“…The choice of therapeutic strategy for nasal reconstruction in the child remains highly controversial [1][2][3][4][5], particularly with respect to the optimal age for performing the procedure. The critical point is whether early or late reconstruction is the better therapeutic choice.…”

“…Revision rhinoplasty rates are higher in children compared to adults . Open rhinoplasty is the preferred technique due to avoidance of interrupting the cartilaginous nasal skeleton . Timing of surgery is controversial, with most surgeons traditionally waiting until females reach age 14 and males reach age 16 to avoid negatively impacting growth .…”
