“…The frequency of incorrect reports probably increased because reporters input the occurrence of an adverse reaction when they observed the symptom listed on PDA without considering carefully whether it was caused by the transfusion or not. Incorrect reports not only make the true frequency of adverse reactions unclear but may also lead to misdiagnosis and wrong treatment . Transfusion specialists should verify the facts together with the reporter.…”
