Onlay versus sublay mesh repair for ventral hernia

Abstract: Background: Ventral hernia repair is one of the most common surgical operations performed all over the world. Onlay and sublay mesh repairs are the commonly performed techniques for the same. However, the debate still continues about the superiority of both techniques over each other. The aim of this study was to compare the outcome of the onlay versus sublay mesh repair for ventral hernia. Methods: A total of 100 patients with paraumbilical, epigastric, supraumbilical and incisional hernias (with defect size ≤… Show more

“…Since these complications were generally wound complications and seroma, it is thought that they were attributable to the more extensive dissection in the abdominal wall for exposure of the anterior rectus sheath and the anterior abdominal wall fascia for mesh placement in the onlay position. The lower recurrence rate in the sublay group was also found in other studies 1% vs 0% by Dhaigude BDet al and 4.35 % vs 8.51% by Raghuveer MNet al 3,13 .…”

“…other studies also supported shorter hospital stay and early drain removal for patients who underwent sublay repair. 3,4,8,10,13,14 Conclusion From our study on a small sample size we can conclude that Sublay mesh repair might be considered to be a better alternative to onlay mesh repair in terms of less seroma formation, less wound infection, early drain removal, less hospital stay and most importantly fewer recurrences, though it takes longer operative time.…”

“…4,5 General surgeons face a huge difficulty due to the high recurrence rate, high cost, and high morbidity associated with incisional hernia surgery. 6 Despite the frequent necessity for surgical repair, physicians continue to struggle to obtain ideal results, and surgical failure is alarmingly prevalent (10-30%). 3 True recurrence rates are almost certainly overstated.…”

“…4 Recurrence, the hernia surgeon's greatest nightmare, escalates healthcare costs and adds further financial hardship on patients. 6 Given that a ventral hernia is caused by the biological issue of stable scar tissue development, mesh techniques are currently the leading therapies for hernia repair. 7 In order to prevent recurrences, facial autografts, prosthetic materials, and various meshes were all employed to assist the repair.…”
