One year outcome of hemi-Descemet membrane endothelial keratoplasty

Abstract: Hemi-DMEK may provide visual outcomes similar to those in standard DMEK at the 1-year postoperative mark. If also graft survival and complication rates prove to be similar, hemi-DMEK could become the next step in endothelial keratoplasty, owing to its potential to double the yield of transplants from the same donor pool (two hemi-DMEK grafts can be prepared from one donor cornea).

“…Furthermore, the scarcity of donor tissue led to endeavours to save donor tissue, for example, by splitting tissue into two parts for so-called hemi-DMEKs (Lam et al 2014(Lam et al , 2015Gerber-Hollbach et al 2016). This seems to be associated with an equally good gain of transparency but a significant drop of ECD postoperatively.…”

“…Our results of alizarin red staining displayed that the ECD of the TE-HCE-transplanted rabbit corneas was 2703 ± 70.37 cells/mm 2 on average 358 days after PKP surgery, while that of control corneas was only 2509.90 ± 51.11 cells/mm 2 . The ECD of the TE-HCE-transplanted rabbit cornea was higher than that of the corneas in TE-HCE-transplanted rabbits and cats in our previous study (Fan TJ et al, 2011;Fan et al, 2013), and much higher than that of the corneas 12 months after endothelial keratoplasty in donated cornea-transplanted patients (<2000 cells/ mm 2 ) (Van Dooren et al, 2004;Kettesy et al, 2014;Lam et al, 2015;Yoeruek et al, 2015). Moreover, our results of H&E staining and SEM and TEM studies revealed that a continuous monolayer of corneal endothelial cells in hexagonal or polygonal morphology attached tightly to the de novo generated DM was reconstructed.…”

“…Although some mechanical damage was observed at the cut edge, as well as at the very limbal edge, the endothelial layer appeared viable up to 5 weeks in organ culture (after which, deterioration of the cellular morphology was seen similar to that in routine culture of full-thickness corneas). Preliminary surgical experience showed that routine DMEK techniques can be used for hemi-DMEK grafts 17 18. In this initial small series of the first consecutive three cases, the visual outcomes of hemi-DMEK appeared to be similar to DMEK, but the endothelial cell decrease with hemi-DMEK was larger,17 18 though a much larger clinical case series would be required for more definitive results.…”

Another current limitation of this technique that might need consideration in the clinical practice is the shape mismatch of the hemi-DMEK graft and the area of the circular descemetorhexis.…”
