One-trocar appendectomy

Abstract: Our results suggest that this technique, which combines the advantages of both the open and the laparoscopic procedures, is a valid alternative for the treatment of acute appendicitis. However, this procedure cannot always be completed using only one trocar, as happened in 10 cases in our series.

“…Single incision laparoscopic appendicectomy, first published by Rispoli et al revolutionised minimal invasive surgery with regard to appendicectomies. 3 In this age of minimal access surgery, there has been a continuous debate among surgeons, on whether Single incision laparoscopic appendicectomy(SILA) is the treatment of choice for this condition. With more patients requesting for affordable day care surgery and better cosmesis in recent times, research work to evaluate the benefits of SILA over the conventional laparoscopic appendicectomy (CLA) has become the need of the hour.…”

“…1 Since the first report of Single incision laparoscopic surgery (SILS) for acute appendicitis by Rispoli et al, it has been proposed as the next revolutionary milestone in minimally invasive surgery. 3 However, at that time evidence supporting the safety and efficacy of this approach was limited. The increased practice of SILS has led surgeons to ponder whether SILS can soon replace conventional laparoscopic surgery.…”

“…Sometimes have proper complications as visceral thermal lesions, vascular trauma or visceral perforation by the trocar 8 . In of context of uncertainty between which treatment is more effective and due to its attractive potential, the video-assisted approach was introduced in teenagers and adults 2,12,16,17,21 . Based on the literature 2,12,14,19,21,28 the method video-assisted provides some attractives, namely: explore all the abdominal cavity providing accurate diagnosis, permits complete and effective cleaning; requires only a laparoscopy diagnostic to locate and externalize the appendix with magnification of image; no need of special materials; no raising in costs; ease to be use in urgency and at night; can be covered by health system in Brazil.…”

“…[3][4][5][6] With the advent of minimal invasive surgery, the number of ports has been reduced to further improve cosmetic outcome. [7][8][9] Single port laparoscopic appendectomy that requires only a single incision is becoming more popular. This surgical approach provides a number of advantages, including improved aesthetics (a single umbilical incision), lower risk of injury to abdominal muscle vessels, less postoperative pain and the possibility of converting to the conventional laparoscopic procedure (combined trocars) if necessary.…”
