One-stage operation for locked bilateral posterior dislocation of the shoulder

Abstract: A man of 52 years of age had a grand mal seizure. He presented to our unit three months later with irreducible bilateral posterior dislocation of the shoulder. CT scans revealed large compression defects on the anteromedial aspect of the heads of both humeri. The defect on the right side was of more than 50% of the articular surface, and on the left side of 40%. He was treated by a one-stage operation with a hemiarthroplasty on one side and reconstruction of the head by an osteochondral autograft on the other.… Show more

“…We think that in such extremely rare cases, with no general contraindications and the patient's consent, simultaneous reduction with fractures stabilization is a solution which should be taken into account. The literature documenting simultaneous bilateral open reduction of posterior and anterior shoulder dislocations and our case with the obtained clinical results substantiate the adopted mana gement method in carefully selected cases [7,16,17]. Our case has also disclosed that clinical improvement may be expected even 2 years after the surgery.…”

“…Posterior shoulder dislocation management depends on the patient´s age, duration of dislocation, extent of damage to articular surface and patient demands and level of activity [6,[12][13][14]17]. The time from the dislocation and the reverse Hill-Sachs lesion size are the major factors in determining whether or not close reduction can be used.…”

“…One way to help to confirm the diagnosis is the unique trauma mechanism: Traumatic posterior shoulder dislocations follow a specific injury mechanism with adduction and internal rotation in an outstretched anteflexed arm [10]. Dislocation duration and size of the humeral head defect are the major factors in determining the treatment plan [6,8,13,14]. Acute dislocation (less than six weeks of injury) and small defect up to 25% of the humeral head articular surface can be treated by closed or open reduction [6,13].…”
