Abstract:As the population ages, the demand for suitable rental housing will increase. Suitable housing means housing that can accommodate those impairments that typically correspond with ageing. This paper explores the quality of life (QoL) requirements of those elderly with high-care needs who live in rental housing. Using a qualitative case study approach, it examines the living experiences of six elderly people who need assistance and are living in local-authority rental housing in New Zealand. The themes of QoL were identified from the literature and related to the larger themes of; 1. Activities and independence, 2. Sense of control, 3. Privacy, 4. Relationships, 5. Quality of care, and 6. Comfort. The survey consisted of a detailed documentation of the physical environment, followed by interviews with and full-day observations of the residents and their caregivers. The study finds that the design of housing that improves their QoL requires solutions to accommodate the various conflicting needs for their QoL that include those derived from the diversity in the user's preferences and impairments. In the design of rental housing, there is greater need for additional or reorganized space to accommodate caregivers and visitors, maintain residents' independence, privacy, and other aspects important for their QoL.

“…Within the arC, some people are concerned with finding and being at home (Cooney 2011), some are concerned with making a living, while for others the arC is just one stop among many in their daily schedules (suppliers, visitors, healthcare professionals). The following section relies upon data reported in two small qualitative research projects exploring aspects of work and daily life within three quite distinct arC facilities (Henley and Jaye 2015;Jaye et al 2015Jaye et al , 2016, and in the report from a Human Rights Commission enquiry into the New Zealand aged care workforce (Human Rights Commission 2012) as well as other relevant New Zealand research. Global processes of cosmopolitanisation contain moral agendas that link macro level structures with local communities, families, individuals, workplaces, and organisations. The aged residential care (ARC) facility as a moral economy exemplifies vernacular cosmopolitanisation. The State shapes the moral economy of the New Zealand aged care sector through enforceable policies and regulations, while organisations and facilities compete in a neoliberal consumption-oriented marketplace. Those employed in the sector such as nurses and careworkers seek a living. Residents must find home within ARC, while their families expect they receive quality care from ARC facilities.

The care of older adults with extreme obesity in nursing homes: A collective case study

Objective: To establish the preparedness of nursing homes to deliver high quality, safe and equitable bariatric care for older adults with extreme obesity. Design and methods: A collective case study approach was used. Data collection included observational and interview data from three nursing homes, and a review of 224,200 resident admissions over a 3-year period in New Zealand. Participants: Twenty eight health care workers from three nursing homes in the North Island of New Zealand. Results: Despite a willingness by healthcare staff to care for older adults with extreme obesity, nursing homes were not well equipped to provide safe equitable care for this resident population. Key areas of concern for nursing homes related to limitations in the infrastructure, and financial barriers relating to government funded contracted care services which incorporated equipment procurement and safe staffing ratios. Conclusion: Nursing homes are unprepared to accommodate the existing and increasing number of older adults with extreme obesity who will require bariatric specific care. Government agencies and policy makers will need to consider the financial implications of the increasing need for bariatric level support within aged care, as well as the impact on individual nursing home resources and quality of care provided. Considerable sector and government attention is needed in relation to infrastructure and funding, to allow for the provision of high quality, safe and equitable care for this population group.
