The popularity of international travel continues to increase among Americans, even though they often experience subsequent illness on return from their journey. The pathogens responsible are not necessarily endemic to the destination itself but are often the result of poor sanitary conditions or activities engaged in while away. Skin disease ranks third among all medical concerns in returning travelers. This review addresses the pathogenesis, epidemiology, clinical presentation, and treatment of the most common skin diseases in returning travelers: insect bites and bedbugs, cutaneous larva migrans, scabies, tungiasis, myiasis, leishmaniasis, viral exanthems, and marine envenomation. Primary care physicians and dermatologists should be familiar with these illnesses and a general approach to their evaluation and management. Key PointsArthropod assault and infestation is a source of significant morbidity in returning travelers.Travelers should bring proper protective clothing and insect repellant when travelling to areas where biting insects are vectors of disease.Travelers should be acquainted with the hazards associated with local marine life if they plan on entering the water.

The agent responsible is any of the three free-swimming or larval stages (ephyrae, medusae, planulae) of the minute “thimble jellyfish” Linuche unguiculata [32] which are easily trapped in or beneath clothing causing nematocyst discharge. Contact with the creature is not painful but a prickling or stinging sensation may be experienced while the victim is in the water followed later by severe itching.…”

Just over a century ago, animal responses to injections of jellyfish extracts unveiled the phenomenon of anaphylaxis. Yet, until very recently, understanding of jellyfish sting toxicity has remained limited. Upon contact, jellyfish stinging cells discharge complex venoms, through thousands of barbed tubules, into the skin resulting in painful and, potentially, lethal envenomations. This review examines the immunological and toxinological responses to stings by prominent species of jellyfish including Physalia sp. (Portuguese Man-o-War, Blue-bottle), Cubozoan jellyfish including Chironex fleckeri, several Carybdeids including Carybdea arborifera and Alatina moseri, Linuche unguiculta (Thimble jellyfish), a jellyfish responsible for Irukandji syndrome (Carukia barnesi) and Pelagia noctiluca. Jellyfish venoms are composed of potent proteinaceous porins (cellular membrane pore-forming toxins), neurotoxic peptides, bioactive lipids and other small molecules whilst the tubules contain ancient collagens and chitins. We postulate that immunologically, both tubular structural and functional biopolymers as well as venom components can initiate innate, adaptive, as well as immediate and delayed hypersensitivity reactions that may be amenable to topical anti-inflammatory-immunomodifier therapy. The current challenge for immunotoxinologists is to deconstruct the actions of venom components to target therapeutic modalities for sting treatment.

SamenvattingIn dit artikel worden enkele huidaandoeningen beschreven die het gevolg zijn van contact met -meestal exotische -fauna en die in Nederland regelmatig bij migranten en als ongenode vakantiegasten bij reislustigen kunnen worden aangetroffen. Achtereenvolgens passeren de revue: myiasis, larva migrans, larva currens, dracunculiasis, tungiasis, seabather's eruption en cercariadermatitis.Door toenemende reislust met een hang naar avontuur verlaten jaarlijks vele Nederlanders hun gerieflijke woning om onder primitieve omstandigheden hun vakantie door te brengen. Vaak wil men contact hebben met de natuur. Dit betekent ook dat de natuur contact maakt met de reiziger. Dat is voor velen een onaangename verrassing. De meest uiteenlopende infecties en pijnlijke beten van de lokale fauna kunnen de vakantiepret bederven. Maar ook kunnen ongemerkt ongenode gasten zich aan ons lichaam opdringen. Enkele daarvan worden hier besproken. Myiasis 1Bij myiasis wordt de huid bewoond door de larven van uiteenlopende soorten vliegen uit de orde der Diptera. Er zijn twee klinische varianten: myiasis in open wonden en furunculeuze myiasis. VerspreidingsgebiedMyiasis wordt wereldwijd aangetroffen. Waar vliegen zijn, komt myiasis voor. Ook in Nederland worden met enige regelmaat maden in wonden aangetroffen, vooral bij wat verwaarloosde personen. In warme streken komen dergelijke infecties vaker voor, vooral wanneer de hygiënische omstandigheden te wensen overlaten. Furunculeuze myiasis wordt met name veroorzaakt door vliegensoorten uit Afrika en Amerika. De dadersStinkende open wonden hebben een aantrekkingskracht op aasvliegen, die graag hun eieren in wonden of in de nabijheid daarvan deponeren. De larven (maden) die uit de eieren komen, voeden zich met vocht en weefsel uit de wond. Meestal eten ze alleen dood weefsel, maar er zijn ook enkele species die zich voeden met vitaal weefsel.
