Background:Propofol has the disadvantage of causing pain or discomfort on injection. The aim of the study was to assess the efficacy of pretreatment with various drugs to alleviate the propofol injection pain.Methods:One hundred American Society of Anesthesiology (ASA) I and II adults, scheduled for various elective surgical procedures under general anesthesia (GA), were included in the study. They were randomly divided into four groups having 25 patients in each group. Group A received pretreatment with intravenous (i.v.) magnesium sulfate, group B received i.v. granisetron, group C received i.v. nitroglycerine and group D was the control group. One-fourth of the total calculated induction dose of propofol was administered over a period of 5 seconds. The patients were asked about the pain on injection. The intensity of pain was assessed using verbal response. A score of 0–3 which corresponds to no, mild, moderate and severe pain was recorded.Results:All the three drugs reduced the incidence and intensity of pain on propofol injection but the order of efficacy in attenuation of pain on the propofol injection was granisetron > nitroglycerine > magnesium sulfate > control.Conclusion:Granisetron was the most effective followed by nitroglycerine and magnesium sulfate in attenuating pain on propofol intravenous injection.

ÖZETAmaç: Propofol, anestezi indüksiyonu, idamesi ve sedasyonda sık kullanılan bir anestezik ajandır. En önemli yan etkisi enjeksiyon ağrısı ve hemodinamik yan etkileridir. Yeni bir formul olan propofol lipuro ile enjeksiyon ağrısının azaldığı belirtilmektedir. Çalışmamızda subanestezik dozda propofol ile propofol lipuronun enjeksiyon ağrısı ve hemodinamik yanıt üzerine etkileri karşılaştırılmıştır.Gereç ve Yöntem: Çalışmaya dahil edilen 150 hastanın yarısına 2 mL % 1 propofol (Grup P) diğer yarısına da 2 mL % 1 propofol lipuro (Grup PL) 1mL sn -1 hızında verildi. Beşinci sn'de ağrı skorları ve hemodinamik bulgular kaydedildi.Bulgular: Grup P'deki hastaların hepsi enjeksiyon ağrısı duyarken, Grup PL'de 17 hasta (% 22,7) hiç ağrı duymadı. Şiddetli ağrı duyan hasta sayısı Grup P'de 44 (% 58,7), Grup PL'de ise 20 (% 26,7) idi (p=0.001). Hemodinamik yanıt her iki grupta benzerdi.Sonuç: Propofol lipuro, daha az enjeksiyon ağrısına neden olması nedeniyle propofole tercih edilebilir. Its most important side effects are injection pain, and hemodynamic side-effects. There are reports stating that with a new formula (Propofol-Lipuro) injection pain is reduced. In our study the effects of sub-anesthetic dose of propofol and Propofol-Lipuro on injection pain and hemodynamic response have been compared. Material and Methods:To 75 patients included in the study, 2 mL 1 % propofol (Group P), and to another 75 cases 2 mL 1 % Propofol-Lipuro (Group PL) were administered at a rate of 1 mL s -1. Pain scores and hemodynamic findings at the postinjection 5 th secs were recorded. Results: While all the patients in Group P felt injection pain, 17 patients in Group PL (22.7 %) did not feel any pain at all (p<0.05). Severe pain was felt by 44 (58.7 %) in patients Group P, and only 20 cases (26.7 %) in Group PL (p<0.05). Hemodynamic response was similar in both groups.Conclusion: Propofol-Lipuro can be preferred over propofol due to its milder injection pain.

Amaç: Propofole bagli enjeksiyon agrisinin giderlmesinde en etkiü ve güvenilir metodun tespit edilmesi amaçlandi. Gereç ve Yöntem: Bu prospektif, randomize, çift kör, placebo kontrollü çaliçmada el bileginden turnike uygunarak pentotal, lidokain, salin (SF), ketamin, ondansetron, dekort verildi. Sonrasinda propofol enjeksiyonu sirasinda kol çekme, yüz buruçturma, vrs ve postop hatirlama oranlan degerlendirildi. Bulgular: Gruplar arasi verbal agri skoru (VRS) verileri degerlendirildiginde agn hissetmeyenlerin sayisinin en fazia lidokain grubunda oldugu (%86) tespit edilmijtir. Lidokaini sirasiyla ondansetron grubu (%82), dekort grubu (%72), pentotal grubu (%52), ketamin grubu (%46) ve SF grubu (%12) takip etmektedir.Sonuç: Propofol enjeksiyon agnsinin giderilmesinde en olumiu sonucu lidokain grubunun gösterdigi belirlenmiçtir. Bununla birlikte ondansetron grubu da propofol enjeksiyon agnsinin giderilmesinde lidokain grubuna yakin sonuçlar vermiçtir. Anahtar kelimeler: Propofol, agri, lidokain ABSTRACT Can propofol injection pain be avoided?Objective: The main purpose of this study is to investigate the most effective and reliable method for prevention of propofol injection pain. Material and Methods: In this prospective, randomized, double-blind trail we gave one of the six drugs of penthotal, lidocaine, saline, ketamine, ondansetron and decort by using wrist tourniquet. Then we evaluated arm withdrawn, grimace, verbal rating scale (VRS) and postoperative recall the pain during propofol injection.Results: Evaluation of VRS data between six groups revealed that the lidocaine group was the most painless group (86%). These findings were followed respectively by ondansetron (82%), decort (72%), penthotal (52%), ketamine (46%) and saline (12%). Conclusion: In this trail we revealed that lidocaine group was the most successful group to handle with propofol injection pain. Furthermore ondansetron group results were similar to lidocaine group.
