Oncologic Outcome of ypT1-2N0 Rectal Cancer After Neoadjuvant Chemoradiotherapy Compared With pT1-2N0 Rectal Cancer

Abstract: Compared with patients who did not receive preoperative CRT, patients who received preoperative CRT did not have significantly different clinicopathologic features except clinical stage and distal resection margin. The 5-year disease-free survival (DFS) rates were lower in patients who received preoperative CRT than those who did not (84.4% vs. 95.5%, P=0.029). Preoperative CRT was a prognostic factor affecting 5-year DFS in patients with pathologically proven stage T1N0 mid and lower rectal cancer (HR, 11.157… Show more

“…Li et al have used propensity score-matching to analyze 168 matched pairs of rectal cancer patients with ypStage I and pStage I, and demonstrated similar prognoses; however, 45.2% of ypStage I patients were treated using AC whereas no pStage I patients received AC, which may have influenced the survival outcomes (27). Moreover, this propensity score matching study and three other reports included only a few (five or less) ypT1 patients (23,24,26,27). To our best knowledge, the current study is the first to compare the prognosis of only rectal cancer patients with pathological T1 stage, including 18 ypT1 patients.…”

“…However, it remains unclear whether rectal cancer downstaged by preoperative CRT has a different prognosis from that of the same stage without preoperative CRT. Several groups have previously conducted comparative studies of patients with pathological stage I lower rectal cancer according to preoperative CRT, but the conclusions regarding their prognoses were inconsistent (23)(24)(25)(26)(27). Moreover, no similar study confined to pT1 stage has been conducted.…”

“…However, the benefits of chemoradiotherapy for OS and CSS was only found in the groups of T2 stage in our study. Furthermore, some studies demonstrated neoadjuvant chemoradiotherapy prior to surgery could not reduce tumor recurrence in early T-stage lower rectal cancer compared with surgery alone [37,38]. This different result may be contributed to the number of patients and subgroup analysis.…”
