Oncogenic osteomalacia due to phosphaturic mesenchymal tumour in the upper thoracic spine

Abstract: Oncogenic osteomalacia (OO) is an uncommon paraneoplastic syndrome occurring due to the presence of a tumour that oversecretes fibroblast growth factor-23, which impairs renal phosphate handling. In most cases, the tumour is a morphologically distinct entity called ‘phosphaturic mesenchymal tumour’ (PMT). Spinal tumours causing OO are exceedingly rare. A 55-year-old man presented with multiple bone pain and proximal muscle weakness in the lower limbs. The constellation of biochemical findings (hypophosphataemi… Show more

“…Cases of PMT were reviewed and summarized in 2020 by Garg et al. ( 5 ). Since its discovery in 1947, only 21 cases of PMT have been reported involving the spinal column, of which only six were PMT involving the thoracic spine.…”

“…In soft tissues, PMT most often involves the extremities, while bone tumors usually involve the appendicular bones, skull, and sinuses (1). Cases of PMT were reviewed and summarized in 2020 by Garg et al (5). “…Garg et al in 2020 highlighted that there have been only 21 cases of TIO-associated mesenchymal tumors involving the spinal column reported in the 70 years since these tumors were first described, of which only six were PMTs involving the thoracic spine. All of these were treated surgically; four without recurrence at follow-up, one with recurrence of serum abnormalities at follow-up, and one with no follow-up documented ( 7 ).…”

“…TIO is commonly located in the craniofacial regions and limbs [1]. Only 20 spine tumor cases have been reported in the literature [4]. The refractory rate of TIO overall was reported to be 18.3% but highest in spine tumors (77.8%) [5].…”
