Oncogenic human papillomavirus is not helpful for cytology screening of the precursor lesions of anal cancers in Taiwanese men who are infected with human immunodeficiency virus

Abstract: Anal cytology yielding atypical squamous cells of undetermined significance or higher grades could detect two-thirds of high-grade anal intraepithelial neoplasias in HIV-infected men and should be promoted for anal cancer prevention.

“…In total, 9 studies reported on the performance of any HR‐HPV test for detecting anal precancers and cancers, including 671 findings of AIN2 or greater and 2154 findings of AIN2 or lower . Overall, the pooled sensitivity and specificity of HR‐HPV for detecting AIN2 or greater was 91.3% (95% CI, 78.9%‐96.7%) and 33.1% (95% CI, 22.2%‐46.3%), respectively (Fig.…”

“…Genotyping for HPV16/HPV18 has the potential to improve the specificity of HPV testing for anal precancer detection. In total, 5 studies reported on the performance of HPV16/HPV18 genotyping for the detection of anal precancers and cancers, including 510 findings of AIN2 or greater and 1562 findings of AIN2 or less . Overall, the pooled sensitivity and specificity of HPV16/HPV18 for detecting AIN2 or greater was 39.9% (95% CI, 22.4%‐60.5%) and 74.3% (95% CI, 67.3%‐80.1%), respectively.…”

“…Previous studies in Taiwan showed that genital HPV was detected in 45.3% of 305 HIV-infected men 13 and that 90% of 196 HIV-infected men had anal HPV, 81% of whom had oncogenic anal HPVs. 14 In another study, the authors demonstrated that 77% of 130 HIV-infected homosexual men had anal HPV infection. 15 In Japan, 75% of HIV-infected MSM were shown to harbour anal oncogenic HPVs.…”

“…Previous studies in western countries have shown that the rate of anal HPV infection in men who had HIV is in the range of 46-92% [11,13,14]. A study conducted in southern Taiwan showed that 77% of 130 MEM were infected with HIV and had anal HPV infections [15], whereas in northern Taiwan, 90.8% of men who had HIV had also tested positive for anal HPV [16].…”
