Oncofertility

Abstract: Adolescents and young adults ages 15 to 39 who are diagnosed with cancer (AYA survivors) undergo a range of therapies for cancer cure, but subsequently may be at risk of treatment-related infertility and, for female AYA survivors, adverse pregnancy outcomes. Future fertility is important to AYA survivors. Meeting their fertility goals requires awareness of this importance, knowledge of cancer treatment-related fertility risks, appropriate fertility counseling on these risks, and access to fertility care. “…However, cervical carcinoma is still the second most common cancer and the most frequent cancer inducing the death of female in 42 middle-and low-income countries (17). A great deal of attention has been paid to the fertility problems of cancer patients at child-bearing age (18), including cervical carcinoma. Although platinum-based chemotherapy strategy, like cisplatin, can alleviate the condition of cervical cancer, it still causes significant side effects (19) and accompanies with obvious reproductive toxicity and genotoxicity, such as a significant decrease and maturation arrest in germ cells (20), oocyte apoptosis in human ovary (21), amenorrhea or azoospermia (22).…”

“…Transplantation of frozen-thawed ovarian cortical tissue was demonstrated to provide recovery of ovarian functions and to restore spontaneous fertility. Successful hormonal stimulation with gonadotropins and in vitro fertilization have been reported in women who underwent retransplantation of ovarian tissue (so far, more than 200 live births worldwide) [ 27 , 86 , 87 , 91 , 92 ]. Initial reports on pregnancies resulting from transplanting ovarian tissue that was sampled and frozen in prepubertal years have also begun to appear [ 93 ].…”

“…Regardless of their affliction, prognosis, and treatment, many AYAs diagnosed with cancer express a desire to have children in the future [ 25 , 26 ]. These patients rank having children among their top three life goals, along with health and education [ 27 ]. The improvement of treatment and survival of pediatric oncology patients has made the preservation of fertility a central survivorship issue [ 19 ].…”
