Objective: to study the dynamics of risk factors for cardiovascular diseases in patients who are under dispensary observation for the main disease arterial hypertension and coronary heart disease; the frequency of visits to a medical organization, depending on the status of dispensary observation in a cohort of adults in 2015 and 2018. Materials and Methods: on the basis of the outpatient department of the Scientific Research Institute – Krasnodar Regional Hospital No. 1, we analyzed the incidence of CVD in the adult cohort (n = 1170), which passed of the clinical examination twice in 2015 and 2018. The analysis was carried out in groups depending on the status of the dispensary observation («not on dispensary observation», «total on dispensary observation» and «consisting on dispensary observation with CVD»). Results: a decrease in unhealthy diet by 32.21%, 28.78% and 28%, respectively, was shown in the groups «not on dispensary observation», «total on dispensary observation» and «consisting on dispensary observation with CVD» in 2015 and 2018, (χ2 = 80.45, p = 0.001, χ2 = 59.96, p = 0.001 and χ2 = 47.8; p = 0.001). In the groups «not on dispensary observation», «total on dispensary observation» and «consisting on dispensary observation with CVD» in 2015 and 2018, there was a decrease in low physical activity by 50.33%, 24.72% and 21.85%, respectively (χ2 = 79.16, p = 0.001, χ2 = 30.6, p = 0.001 and χ2 = 22.42; p = 0.001). Against the background of the ongoing preventive measures in 2015 – 2018. the cohort showed an increase in overweight and obesity by 19.2% in the «not on dispensary observation» group (χ2 = 4.84, p = 0.028); an increase in the frequency of elevated blood pressure by 48.33% in the «not on dispensary observation» group and a decrease in its frequency in the «consisting on dispensary observation with CVD» by 24% (χ2 = 19.77, p = 0.001, χ2 = 7.1; p = 0.008). Against the background of the ongoing preventive measures from 2015 to 2018, a tendency to reduce the frequency of FR CVD was revealed. A significant decrease in unhealthy diet (p = 0.001), low physical activity (p = 0.001), was registered in individuals who were on dispensary observation according to the main nosology of hypertension. In the group of individuals who were on dispensary observation according to the main nosology, there was a significant decrease in unhealthy diet (p = 0.006) and low physical activity (p = 0.023). In patients who are on dispensary observation with CVD, there was a favorable dynamics in terms of achieving the target values of the level of blood pressure and cholesterol. The revealed tendency to decrease the frequency of visits due to CVD in patients «consisting on dispensary observation» by 32.2%, including» consisting on dispensary observation with CVD» by 33.8% for the period 2015 – 2018. Conclusions: there was a tendency to decrease the frequency of risk factor CVD and the frequency of visits due to CVD in patients «consisting on dispensary observation», which indicates the effectiveness of measures carried out within the observation, regardless of the nosological group of patients.

Against the background of the ongoing preventive measures in 2015 – 2018. the cohort showed an increase in overweight and obesity by 19.2% in the «not on dispensary observation» group (χ2 = 4.84, p = 0.028); an increase in the frequency of elevated blood pressure by 48.33% in the «not on dispensary observation» group and a decrease in its frequency in the «consisting on dispensary observation with CVD» by 24% (χ2 = 19.77, p = 0.001, χ2 = 7.1; p = 0.008). Against the background of the ongoing preventive measures from 2015 to 2018, a tendency to reduce the frequency of FR CVD was revealed. A significant decrease in unhealthy diet (p = 0.001), low physical activity (p = 0.001), was registered in individuals who were on dispensary observation according to the main nosology of hypertension. In the group of individuals who were on dispensary observation according to the main nosology, there was a significant decrease in unhealthy diet (p = 0.006) and low physical activity (p = 0.023). In patients who are on dispensary observation with CVD, there was a favorable dynamics in terms of achieving the target values of the level of blood pressure and cholesterol. The revealed tendency to decrease the frequency of visits due to CVD in patients «consisting on dispensary observation» by 32.2%, including» consisting on dispensary observation with CVD» by 33.8% for the period 2015 – 2018. Conclusions: there was a tendency to decrease the frequency of risk factor CVD and the frequency of visits due to CVD in patients «consisting on dispensary observation», which indicates the effectiveness of measures carried out within the observation, regardless of the nosological group of patients.
Objectives to assess the effect of the gender of patients on the severity of risk factors for the development of cardiovascular diseases using specialized software. Material and methods. In order to identify and correct the severity of risk factors for the development of cardiovascular diseases in the population, in 2018-2019, the data of residents of the Republic of Tatarstan were registered in specialized software. The severity of risk factors was assessed before the start of the study and after one year. The research included two groups of persons: men and women. Results. In 2018, the program "Cardiovascular Risk Management" had registered the data of 32,746 residents of the Republic of Tatarstan aged from 18 to 80 years. The risk factors for the development of cardiovascular diseases were identified in 22,999 individuals, where there were 7,188 men and 15,712 women. The patient's data included gender, age, history of coronary artery disease, smoking, physical activity, blood pressure, the levels of cholesterol, glucose and blood creatinine. The data on the severity of the risk factors in both groups were statistically processed. Based on the data obtained, the supervised persons received recommendations for correcting their lifestyle. In 2019, an assessment of the severity of risk factors was carried out in persons included in the system in 2018. Conclusion. We found a statistically significant difference in the results of managing risk factors in men and women. The software "Management of cardiovascular risks" is recognized as an effective tool for managing risk factors for the development of cardiovascular diseases and reducing mortality from diseases of the circulatory system.
Purpose. Evaluation of the effectiveness of risk management for the occurence of cardiovascular diseases residents of municipal districts of the Republic of Tatarstan using the specialized software module “Cardiovascular Risk Management” in dynamics. Materials and methods. The study was based on the cities of Zelenodolsk and Nizhnekamsk in the Republic of Tatarstan. The sample consisted of nine thousand seven hundre twenty-five residents of Nizhnekamsk (6332 (65.1%) women with an average age of 64.1 years, men - 3393 (34.9%) people with an average age of 62.7 years. 4175 people (including 2893 women aged of 63.3 years in average (69.3%), and 1282 men at the average age of 59.7 (30.7%) years. Information and software module “Cardiovascular Risk Management”, which is a medical software product aimed at managing an assessment of the risk of developing cardiovascular diseases in a particular patient and taking preventive measures. Results. The most significant risk factors in residents of both districts were arterial hypertension. In the considered areas, a significant decrease in the severity of risk factors such as “systolic” and “diastolic” blood pressure, cholesterol and glucose levels in residents of the Zelenodolsk municipal district and a decline in the severity of one risk factor in the form of “diastolic” blood pressure in residents of the Nizhnekamsk municipal district were revealed in dynamics. Limitations. 9725 people from two municipal districts of the Republic of Tatarstan took part in the study. The analysis of the effectiveness of the risk management for the occurrence of cardiovascular diseases was carried out. The age-gender distribution is representative of the demographic indicators of the region’s population. Conclusion. The use of the information and software module “Cardiovascular Risk Management” allows effectively identifying existing risk factors in the population and carrying out timely preventive measures, as well as increase the efficiency and quality of the provision of medical and diagnostic assistance to the population living in remote areas of the subject of the Russian Federation.
