Empirical studies that assess which items of the Youth Self‐Report (YSR) are the best predictors of anxiety disorders in adolescents are lacking, whereas several attempts have been made to construct an anxiety scale for the YSR. It is important to gap the bridge between existing YSR and DSM‐IV diagnostic constructs. My objective in this study was to investigate which YSR items or scales can be used best to predict anxiety disorders in adolescents. In a referred 183‐subject sample of 11‐ to 18‐year‐olds, we examined which YSR items and scales predicted DSM‐IV anxiety diagnoses assessed with the Anxiety Disorders Interview Schedule for Children (ADIS‐C). Several items that predicted anxiety disorders were not included in existing anxiety scales of the YSR. Scale scores based on these items were stronger predictors of DSM‐IV anxiety disorders than existing anxiety scales. Testing empirically which YSR items predict DSM‐IV diagnoses may enhance the usefulness of the YSR for predicting anxiety disorders in clinical practice. BACKGROUND AND OBJECTIVES:Childhood and adolescent anxiety is generally associated with a varied somatic symptom pattern thought to reflect autonomic system activity. Few studies have examined the autonomic characteristics of generalized anxiety disorder (GAD). This omission is at odds with contemporary models of autonomic cardiovascular control. The current study aimed to find differences in autonomic functions between children with a diagnosis of childhood anxiety disorder and a control group using a case-control design.DESIGN AND SETTING:A cross sectional experimental study conducted in the years 2004-2005 in the psycho-physiology lab of a tertiary care multi-speciality teaching hospital.METHODS:Assessments were carried out using a semistructured interview, K-SADS (Schedule for Affective Disorders and Schizophrenia for Children and Adolescents); STAIC (State and Trait Anxiety Inventory for Children); CDRS (Childhood Depression Rating Scales); SCARED (Self-Report for Childhood Anxiety–Related Disorders). Autonomic reactivity was tested using the standard battery of tests.RESULTS:There were differences between 34 children and adolescents (age range, 8-18 years) with a diagnosis of childhood anxiety disorder and a control group of 30 age- and sex-matched subjects from a nearby school in autonomic activity and reactivity between individuals with anxiety disorder and non-anxious control subjects. Our finding is suggestive of autonomic rigidity or diminished physiologic flexibility in children with anxiety disorderCONCLUSIONS:The study is probably the first of its kind to look into the issue in detail using a detailed battery of the autonomic function tests, and the results are of help in better understanding the condition. The result of the present experiment supports differences in autonomic activity and reactivity between individuals with anxiety disorder and non-anxious control subjects.

Background: Generalized anxiety disorder (GAD) has undergone a series of substantial classificatory changes since its first inclusion in DSM-III. The majority of these revisions have been in response to its poor inter-rater reliability and concerns that it may lack diagnostic validity. This article provides options for the revision of the DSM-IV GAD criteria for DSM-V. Method: First, searches were conducted to identify the evidence that previous DSM Work Groups relied upon when revising the DSM-III-R GAD and the overanxious disorder classifications. Second, the literature pertaining to the DSM-IV criteria for GAD was examined. Conclusions: The review presents a number of options to be considered for DSM-V. One option is for GAD to be re-labeled in DSM-V as generalized worry disorder. This would reflect its hallmark feature. Proposed revisions would result in a disorder that is characterized by excessive anxiety and worry generalized to a number of events or activities for 3 months or more. Worry acts as a cognitive coping strategy that manifests in avoidant behaviors. The reliability and validity of the proposed changes could be investigated in DSM-V validity tests and field trials.
