On the histogenesis of mixed germ cell-sex cord stromal tumour of the gonads

Abstract: In this investigation, we provide further evidence that the germ cells in testicular MGC-SCSTs are neoplastic; however, in the great majority of tumours, these cells are low-grade. Some testicular MGC-SCSTs arise from an intratubular component. We believe that the majority of ovarian and some testicular MGC-SCSTs arise more directly from simultaneous transformation of germ cells and sex cord derivatives.

“…Although the existence of the entity of mixed germ cell-sex cord stromal tumor (MGC-SCST) of the testis has been challenged, the most recent evidence supports its existence; however, testicular neoplasms differ significantly in histological appearance and clinical behavior from ovarian examples due to both genetic and epigenetic factors [6,7].…”

“…It has been proposed that it can originate from the simultaneous transformation of germ cells and sex cord derivatives, sometimes with the formation of imperfect follicular structures. Roth and Cheng describe 12 cases originating from the ovary [5].…”

“…Nevertheless, the concept of testicular MGC-SCST versus that of sex cord-stromal tumor with entrapped germ cells was not addressed in the latest WHO classification of tumors of the testis [23]. Since that time, additional evidence supporting the existence of testicular MGC-SCST has been presented [24].…”
