“…Among those with negative/nominal results, treatment of the emotional distress can be expected to decrease angina, psychosocial/occupational disability, and inappropriate health system utilization (Cannon et al, 1994;Klimes et aL, 1990) and may prevent evolution of acute cardiac disease in some of these patients (Haines et aL, 1987;Kawachi et al, 1994a, b). Among those with positive results, treatment of emotional distress can be expected to decrease morbidity and mortality (Ketterer, 1993) and should also diminish inappropriate health care utilization (Allison et al, 1995;Ketterer et al, 1994a), although the pertinent mechanisms remain in debate (Frank and Smith, 1990;Lown, 1987;Kamarck and Jennings, 1991;Ketterer, 1991;Maercklein, 1991, 1992). Future work in this area should be directed toward identifying cost-efficient means of screening angiographic patients for emotional distress.…”

“…It is difficult to see how such an effect can occur unless chronic emotional arousal is invoked (Frank & Smith, 1990; Kamarck & Jennings, 1991;Ketterer, 1991;Lown, 1987), although the mechanisms may involve malignant psychobehavioral influences uncontrolled in this study rather than direct psychoneuroendocrine pathways (Ketterer, 1991). One such mechanism may be treatment-seeking delay (Barsky et al, 1991;Kenyon et al, 1991).…”

“…While the psychoneuroendocrine and psychobehavioral mechanisms relating psychosocial/emotional stress to chronic and acute ischemic heart disease are under continuing investigation (Frank & Smith, 1990; Kamarck & Jennings, 1991;Ketterer, 1991;Lown, 1987), most epidemiological investigations of psychosocial risk factors have focused on the aggravation, irritation, anger and impatience I'AIAI') of the Type A Behavior Pattern (Dembroski, MacDougall, Costa, & Grandits, 1989;Friedman, 1988).…”
