The relationship between the hemostatic system and the severity of toxemia was studied in 78 pregnant women with toxemia. The activities of plasma antithrombin III (AT-III), prekaUikrein, plasminogen, Α₂-plasmin inhibitor and factor XIII were determined using the chromogenic substrate and fluorescent methods. The antigens of both AT-III and factor XIII were determined by the single radial immunodiffusion method. Plasma bradykinin was determined by radioimmunoassay. The main results obtained were as follows. Both activities of AT-III and plasma prekaUikrein decreased as the gestosis index increased (p < 0.001), and a significant negative correlation was observed between the total score of the gestosis index and AT-III (r = -0.447, p < 0.005) or plasma prekaUikrein (r = -0.434, p < 0.005). The levels of plasminogen, Α₂-plasmin inhibitor and factor XIII decreased and plasma bradykinin increased in toxemia. Among the various factors, plasma AT-III and prekaUikrein were the most sensitive indicators of the severity of toxemia.

Correlation between Grade III Placenta and Plasma Antithrombin III Activity in Full Term Pregnancy

We evaluated whether the presence of a grade III placenta correlates with blood hypercoagulability in pregnancy between 37 and 39 weeks of gestation. The placenta was graded by ultrasound in 155 healthy full-term women and the plasma levels of antithrombin III (AT III) activity, thrombin-antithrombin complex (TAT) and D-dimer were correlated with each placental grade. AT III activity levels tended to decrease with advancing placental grade from I to III (p < 0.05). D-dimer showed the same tendency while TAT did not. The incidence of reduced AT III activity levels (<70%) in women with a grade III placenta was about twice those in women with a grade II or I placenta, and that of AT III <80% was 3-fold greater. We concluded that the presence of a grade III placenta in full-term pregnancies correlates with blood hypercoagulability.

Summary: Fifty-six pregnant women (gestational age 6-40 weeks) were evaluated for their coagulation activation (fibrin monomers and thrombin-antithrombin III complex) and for their fibrinolysis profile by determining tissue plasminogen activator, plasminogen activator inhibitor, plasminogen, a 2 -antiplasmin-and D-dimer. Fibrin monomers and thrombin-antithrombin III complexes were found to be significantly increasing with gestational age. All the fibrinolytic parameters showed a steady growth with the progress of the pregnancy, with the exception of tissue plasminogen activator which showed a significant decrease with gestational age, but mainly within the reference ränge. These results suggest a Stimulation of the coagulation System and an activation of fibrinolysis with ongoing pregnancy, although the increasing oc 2 -antiplasmin and plasminogen levels and the decreasing tissue plasminogen activator concentrations do not conform to this trend.
