Maternal breast milk (BM) is a complex and unique fluid that evolution adapted to satisfy neonatal needs; in addition to classical nutrients, it contains several bioactive components. BM characteristically shows inter-individual variability, modifying its composition during different phases of lactation. BM composition, determining important consequences on neonatal gut colonization, influences both short and long-term development. Maternal milk can also shape neonatal microbiota, through its glycobiome rich in Lactobacilli spp. and Bifidobacteria spp. Therefore, neonatal nourishment during the first months of life seems the most important determinant of individual's outcomes. Our manuscript aims to provide new evidence in the characterization of BM metabolome and microbiome, and its comparison to formula milk, allowing the evaluation of each nutrient's influence on neonatal metabolism. This result very interesting since potentially offers an innovative approach to investigate the complex relationship between BM components and infant's health, also providing the chance to intervene in a sartorial way on diet composition, according to the nutritional requests. Future research, integrating metabolomics, microbiomics and stem cells knowledge, could make significant steps forward in understanding BM extraordinary properties and functions.

Human microbiota is the aggregate of microorganisms that reside in our body. Its phylogenetic composition is related to the risk for suffering from inflammatory diseases and allergic conditions. Humans interact with a large number and variety of these microorganisms via the skin and mucous membranes. An immune protection mechanism is the production of secretory IgA (SIgA), which recognizes resident pathogenic microorganisms and prevents their interaction with host epithelial cells by means of immune exclusion. Formerly, it was thought that SIgA only function in mucous membranes was to recognize and exclude pathogens, but thanks to the use of massive sequencing techniques for human microbiota phylogenetic characterization, now we know that it can be associated with pathogenic and non-pathogenic microorganisms, an association that is important for functions the microbiota carries out in epithelia, such as regulating the capability of certain microbial species to settle on the skin and mucous membranes, and stimulation and regulation of the immune response and of the risk for the development of inflammatory problems, allergic conditions, autoimmune diseases, and even cancer. Established microbiota determines the type of bacterial species (and probably viral and protozoan species) that reside on the skin and mucous membranes, promoting microbial diversity.

A prospective clinical study of Primo-Lacto: A closed system for colostrum collection

BackgroundColostrum is the first nutritional liquid that comes out of the breast during lactation. Colostrum collection can be challenging due to the small volume produced, and because breast pumps are not designed for colostrum collection. Besides pumping colostrum, the generally accepted practice is to use any available container to hand-express colostrum. Transfer between containers may lead to contamination, higher chance of infection and loss of colostrum. Our aim was to understand if a dedicated colostrum collection system (Primo-Lacto, Maternal Life, LLC, Palo Alto, CA) is more effective than standard hospital practice.MethodsMothers who delivered preterm infants < 34 weeks gestation and mothers with non-latching infants were approached within 24 hours of delivery. Surveys were distributed to participating patients (n = 67), and nurses or lactation consultants (n = 89). Mothers compared ease of use, their confidence level and satisfaction with the amount collected during standard practice vs. the colostrum collection system. Nurses or lactation consultants compared ease of use, differences in colostrum loss and time invested collecting. Quantitative data were analyzed using the Wilcoxon signed rank test and qualitative data were analyzed with grounded theory methods.ResultsFor mothers, ease of use and confidence were significantly better when they used the colostrum collection system than when they used the standard collection procedure, and this difference was true for both hand and pump expression (p<0.01). Nurses and lactation consultants perceived that ease of use was better, and percent of colostrum lost was significantly less with the colostrum collection system for both hand and pump expression. The collection times were not significantly different between the colostrum collection system and standard practice.ConclusionThe colostrum collection system is a tool to help facilitate successful colostrum collection and improve the experience both for clinicians and patients.
