Olfactory Sensations During Proton and Photon Radiotherapy: A Multicenter Prospective Observational Study

Abstract: Purpose: Patients often report a sense of smell during radiation therapy (RT), but the details of these events are not well understood. The purpose of the study was to evaluate events of smell during photon RT and proton beam therapy (PBT).Methods and materials: The subjects were all adult patients (≥20 years old) treated with photon RT or PBT at two centers from January 2019 to August 2020, with the exclusion of those with communication difficulties or olfactory abnormality. The presence of smell, odor type, … Show more

“…Our recent research showed the patients who received total body irradiation (TBI) felt odor (14 of 32 patients) and 2 of 14 patients answered “ozone” as the type of smell without knowledge of ozone as a possible cause ( 26 ). These reports indicate that ozone may be cause of the odor, however various types of odors have been reported in these reports as well ( 26 , 27 ), and it is not possible to identify the cause of the odors as ozone alone.…”

“…There is less information about odor, but this may be caused by a small amount of ozone generated by irradiation. Ozone is often described as having a burnt or chemical odor, and these odors were common in and 2 of 14 patients answered "ozone" as the type of smell without knowledge of ozone as a possible cause (26). These reports indicate that ozone may be cause of the odor, however various types of odors have been reported in these reports as well (26, 27), and it is not possible to identify the cause of the odors as ozone alone.…”

“…In previous studies, 27% of patients in whom the nasal cavity was included in the radiation field sensed odors and only 5% of patients in whom the nasal cavity was not included in the radiation field sensed odors [ 2 ]. An odor was sensed in 44% of TBI cases in the current study, which is a higher rate than that in radiotherapy other than TBI.…”

“…Visual abnormality was defined as unable to distinguish the colors on the chart with or without glasses, and abnormal olfactory sensation was confirmed by interview. A checklist was used with the same format as in previous studies to evaluate light and odor during TBI [ 1 , 2 ]. TBI was performed at 12 Gy in 6 fractions for 3 days, and TBI on the same day was performed with an interval of at least 6 hours.…”
