Chemosensory disorders are a possible disturbance in Sjögren’s syndrome (SS). The aim of the study is to comprehensively present chemosensory disorders in SS and to indicate their possible causes. The possible causes of taste and smell disorders in SS are changes in the structure of exocrine glands and their dysfunction, damage to receptors and weakening of their ability to regenerate, and neurological changes in the form of peripheral neuropathy and impaired cognitive function. Other postulated causes of chemosensory disorders are autoimmune mechanisms, adverse effects of drugs used in SS, and primary potentially SS-triggering viral infections. They are multifactorial and may occur independently of each other. The time of their onset and correlation with other disease symptoms may facilitate the determination of their primary cause in each patient. Awareness of chemosensory disorders in SS may help to ease their progress and eliminate other factors responsible for their more severe manifestation. In the prevention and treatment of chemosensory disorders in SS, the most important thing is to alleviate xerostomia and dryness in the nasal cavity and their effects in the form of chronic local inflammations, counteract receptor atrophy, and an implementation of appropriate neurological diagnosis and treatment.

Purpose The purpose of this study was to conduct a large-scale genome-wide association study (GWAS) and construct a polygenic risk score (PRS) for risk stratification in patients with dry eye disease (DED) using the Taiwan Biobank (TWB) databases. Methods This retrospective case-control study involved 40,112 subjects of Han Chinese ancestry, sourced from the publicly available TWB. Cases were patients with DED ( n = 14,185), and controls were individuals without DED ( n = 25,927). The patients with DED were further divided into 8072 young (<60 years old) and 6113 old participants (≥60 years old). Using PLINK (version 1.9) software, quality control was carried out, followed by logistic regression analysis with adjustments for sex, age, body mass index, depression, and manic episodes as covariates. We also built PRS prediction models using the standard clumping and thresholding method and evaluated their performance (area under the curve [AUC]) through five-fold cross-validation. Results Eleven independent risk loci were identified for these patients with DED at the genome-wide significance levels, including DNAJB6 , MAML3 , LINC02267 , DCHS1 , SIRPB3P , HULC , MUC16 , GAS2L3 , and ZFPM2 . Among these, MUC16 encodes mucin family protein. The PRS model incorporated 932 and 740 genetic loci for young and old populations, respectively. A higher PRS score indicated a greater DED risk, with the top 5% of PRS individuals having a 10-fold higher risk. After integrating these covariates into the PRS model, the area under the receiver operating curve (AUROC) increased from 0.509 and 0.537 to 0.600 and 0.648 for young and old populations, respectively, demonstrating the genetic-environmental interaction. Conclusions Our study prompts potential candidates for the mechanism of DED and paves the way for more personalized medication in the future. Translational Relevance Our study identified genes related to DED and constructed a PRS model to improve DED prediction.

Nesse contexto, evidencia-se o estudo de Tashbayev et al (2017) que registrou minuciosamente as características da interdisciplinaridade entre a Odontologia e a Medicina na especialidade reumatológica, em decorrência da associação direta entre sintomas orais e oculares com a SS-p. No aspecto periodontal, segundo os resultados aproximadamente metade da população de Taiwan portadora de SS foi diagnosticada com a doença (Lin et al, 2019), ressaltando que os médicos e cirurgiõesdentistas devem estar atentos aos sinais e sintomas de SS em pacientes com DP, como boca e olhos secos.…”

A Síndrome de Sjögren é uma alteração de origem autoimune, de caráter crônico que afeta principalmente o epitélio das glândulas lacrimais e salivares. As principais características clínicas são a secura ocular (xeroftalmia), sensação de boca seca (xerostomia) e redução do fluxo salivar. Este estudo teve por objetivo, por meio de uma revisão sistemática de literatura, caracterizar a Síndrome de Sjögren, incluindo alterações secundárias de doenças concomitantes que impactam na qualidade de vida do paciente, avaliar de forma interdisciplinar as manifestações sicca (xeroftalmia e xerostomia), relatar a inter-relação da microbiota oral e a patogênese da síndrome, além de descrever as manifestações intraorais, oftalmológicas e sistêmicas. Para obter os dados necessários realizou-se uma pesquisa bibliográfica nas bases de dados PUBMED com os seguintes descritores MeSH: “Sjögren's Syndrome” and "dentistry", "Ophthalmology" estes foram combinados através do operador booleano "AND" e SCIELO com a palavra-chave “Sjögren's Syndrome”. Foram incluídos apenas artigos originais, nos idiomas português, inglês e espanhol, considerando o período de 2016 a 2022. No total, 655 estudos foram encontrados, sendo que destes, apenas 13 foram selecionados por estarem de acordo com os critérios de inclusão. Os estudos analizados demonstraram que a Síndrome de Sjögren demanda um manejo multidisciplinar, no qual é de extrema importância a interação entre Reumatologista, Odontólogo e oftalmologista, contribuindo para o diagnóstico precoce, o controle dos sinais e sintomas e para promover o tratamento individualizado, proporcionando qualidade de vida a esses pacientes.
