Oki stenting for anastomotic bronchomalacia in lung transplantation: Figure 1:

Abstract: Anastomotic airway complications are a frequent cause of disease in lung transplantation. However, there is no consensus on the type of treatment to be performed with prosthetic devices. While some recent gadgets such as the Oki stent have been proposed for main right bronchus stenosis, there are no reports of stenting using this prosthesis in cases where the main complication is malacia rather than stenosis. We present 2 patients diagnosed with main right bronchus bronchomalacia, also involving bronchius inte… Show more

“…Dahlqvist et al reported successful implementation of the Oki silicone stent in two cases with intermediary bronchus stenosis after lung transplantation . Another study also reported successful use of the Oki stent in bronchomalacia in the right lung in two cases; these were also transplant cases . As far as we know, these three reports are the only papers published on Oki stent implementation.…”

“…One advantage of the Oki stent is its right upper lobe arm, which prevents migration. Lopez‐Padilla et al reported two cases where other stent types were tried and failed prior to using an Oki stent . For this reason, they suggest using the Oki stent as the first choice in these cases.…”

“…Owing to easy removal, durability and low cost, the silicone stent is widely used clinically [ 13 , 18 20 ]. However, metallic stents show good support and flexibility, can be placed with the help of guide wires, may be less traumatizing and minimize the procedures in cases with a fistula [ 14 ].…”

“…However, the main carinal angle differs from the primary right carinal angle, so when we insert a Y-stent on RC1, the proximal and distal orifice of the limbs of the Y-stents must be cut obliquely so that each orifice faces the bronchial lumen but not the bronchial wall [6]. To overcome this limitation, a dedicated bifurcated silicone stent designed to fit on the RC1 was developed, and its clinical efficacy has been reported [11,15,16]. The number of patients with indications for placing bifurcated stents on RC1 is higher than that for stents on LC2 in our clinical practice.…”
