Office‐based sclerotherapy for benign parotid lymphoepithelial cysts in the HIV‐positive patient

Abstract: While multiple cysts may arise and serial intervention may be required, we believe that in association with broader clinical experience, the data presented here support aspiration and sodium morrhuate injection as a safe, minimally invasive, and efficacious technique in the treatment of benign lymphoepithelial cysts of the parotid gland in the HIV-positive population.

“…Ehrenstein et al recommend the initiation of combination ART, including 2 nucleoside reverse transcriptase inhibitors, with either a protease inhibitor or a nonnucleoside reverse transcriptase inhibitor. Based on our review, the average complete response of patients receiving ART was approximately 50% with a range from 16.7% to 90% (Table ). Syebele and Bütow reported the highest rates of complete response at 90% with combination HAART (stavudine, lamivudine, and efavirenz).…”

“…The studies on ethanol only evaluated patients subjectively as being pleased or displeased with their results, and therefore will not be discussed. 12 Doxycycline injection into the cysts resulted in a 22% (2/9 cysts) complete response rate 22. Two of the patients in the doxycycline treatment group opted for superficial parotidectomy as definitive treatment for their cysts 13.…”

“…Management consists of needle aspiration; because the cysts have a propensity to recur, sclerotherapy with tetracycline or doxycycline has been shown to be effective. Recently, additional injectable agents (eg, sodium morrhuate) were identified as being effective [10,11]. For small asymptomatic cysts, observation is perfectly appropriate.…”
