ObjectiveThe WHO Commission on Social Determinants of Health (SDH) has called for a health workforce trained in recognising, understanding and acting on the SDH. However, little is known about how current medical education prepares graduates for this challenge. This study analyses the extent to which the German medical education incorporates content on SDH.DesignFollowing a published protocol, in 2018, we conducted a qualitative and quantitative content analysis of three key document groups, defining and guiding what medical schools are expected to teach and what medical students are expected to know when graduating in Germany. We developed the coding system in a mixed inductive and deductive approach based on key WHO documents.SettingMedical schools and the medical education system in Germany.ResultsImportant gaps exist in the representation of SDH in medical education in Germany. Between 3% and 27% of the analysed document-elements made reference to SDH and only 0%–3% of those document elements made explicit references to SDH. While some aspects were covered widely (eg, topics of occupational health, early childhood development and hygiene), other topics such as health inequalities or determinants outside of the healthcare system were not or hardly represented.ConclusionsA stronger and more explicit representation of SDH in German medical education is needed to prepare the new health workforce for current and future challenges in our globalised world and for medical schools to be socially accountable.

As in many European countries, the Public Health Service (PHS) in Germany has had considerable difficulties in attracting well-qualified personnel for decades. Despite ongoing political and societal debate, limited empirical research on possible causes and explanations is available. To identify areas of action, we explored reasons for the (lack of) interest in working in the PHS by conducting two cross-sectional surveys among 3019 medical students (MS), public health students, and students from other PHS-relevant fields (PH&ONM) in Germany right before (wave 1, 2019/2020) and during the COVID-19 pandemic (wave 2, 2021). While interest in working in the PHS among MS was low, it was considerably higher among PH&ONM. The prevalent underestimation of the importance of public health and low levels of knowledge about the PHS were identified as potential barriers. Although core activities of the PHS were often considered attractive, they were repeatedly not attributed to the PHS. A negative perception of the PHS (e.g., it being too bureaucratic) was prevalent among students with and without PHS interest, indicating that both a negative image and potentially structural deficits need to be overcome to increase attractiveness. Based on the findings, we propose approaches on how to sustainably attract and retain qualified personnel.

ZusammenfassungDie COVID-19-Pandemie hat vielfältige Herausforderungen für Public Health und den Öffentlichen Gesundheitsdienst (ÖGD) in Deutschland offenbart bzw. verstärkt. Sie bietet jedoch auch ein Gelegenheitsfenster für eine langfristig wirksame Transformation im Bereich der öffentlichen Gesundheit. Vor diesem Hintergrund erfolgte im Oktober und November 2020 eine Online-Befragung der Mitglieder des Nachwuchsnetzwerkes Öffentliche Gesundheit (NÖG), in welcher die Erfahrungen mit und der Blick auf Public Health während der COVID-19-Pandemie eruiert wurden und sich erste Erkenntnisse aus der Pandemie für den deutschen Public-Health-Kontext abzeichneten. In diesem Beitrag werden diese vorgestellt und ausgehend von den Ergebnissen der Befragung Desiderate formuliert, welche zielgerichtete und konkrete Anhaltspunkte für die Weiterentwicklung und Förderung der öffentlichen Gesundheit geben sollen. Zentrale Themen, welche die befragten Nachwuchsfachkräfte beschäftigten, waren die erhöhte öffentliche und politische Aufmerksamkeit für öffentliche Gesundheit mit einer Fokussierung auf den Infektionsschutz, der Stellenwert von Public Health in Deutschland sowie Stärken und Schwächen von Public-Health-Strukturen und -Fachkräften. Die Desiderate zielen auf eine langfristige und holistische Stärkung von Public Health ab, in der die Ausbildung interdisziplinärer Nachwuchsfachkräfte einen hohen Stellenwert einnimmt.
