Off-the-Shelf Fenestrated Endografts: A Realistic Option for More Than 70% of Patients With Juxtarenal Aneurysms

Abstract: Standardized fenestrated designs suitable for endovascular treatment of >70% of patients with juxta- and pararenal aneurysms currently treated with custom-made fenestrated endografts will soon be available. This new generation of endografts will permit rapid treatment of a large majority of patients requiring fenestrated endograft repair.

“…Such characteristics might be factors limiting the use of some CeT celiac trunk, SMA superior mesenteric artery, AB aortic bifurcation Surg Radiol Anat fenestrated stent-grafts in EVAR in particular off the shelf stent-grafts. As a matter of fact, RRA was the primary cause of non-feasibility among the 28 % of patients unfit for off the shelf stent-grafts for juxta and pararenal aortic aneurysms in Sobocinski et al study [24]. Such anatomical features could require increased focus on renal arteries during preoperative course.…”

“…4 We sought to explore the impact of this new design on the time needed for target vessel catheterization and other relevant procedure variables in comparison to the standard FSGs for the treatment of PAAs, which by definition have at least one renal artery involved in the aneurysm. 6 …”

“…These devices fit most patients requiring a custommade endograft (23). In a retrospective study of 100 consecutive patients, the use of a fenestrated device allowed endovascular treatment in 72% of patients (24). The use of standardized "off-the-shelf" devices could reduce technical difficulties related to preoperative planning and the deployment procedure making this technique feasible even in centers with less endovascular experience.…”
