This study evaluates the role of design, operational, and physicochemical parameters of constructed wetlands (CWs) in the removal of pharmaceuticals (PhCs). The correlation analysis demonstrates that the performance of CWs is governed by several design and operational factors (area, depth, hydraulic loading rate, organic loading rate, and hydraulic retention time), and physicochemical parameters (dissolved oxygen, temperature, and pH); the removal efficiency of about 50% of the examined PhCs showed a significant correlation with two or more factors. Plants contributed significantly in the removal of some of the PhCs by direct uptake and by enhancing the process of aerobic biodegradation. The use of substrate material of high adsorption capacity, rich in organic matter, and with high surface area enhanced the removal of PhCs by adsorption/sorption processes, which are the major removal mechanisms of some PhCs (codeine, clarithromycin, erythromycin, ofloxacin, oxytetracycline, carbamazepine, and atenolol) in CWs. Although the removal of almost all of the studied PhCs showed seasonal differences, statistical significance was established in the removal of naproxen, salicylic acid, caffeine, and sulfadiazine. The effective PhCs removal requires the integrated design of CWs ensuring the occurrence of biodegradation along with other processes, as well as enabling optimal values of design and operational factors, and physicochemical parameters.

Emerging pollutants are daily released into water bodies, such as pharmaceuticals released by urine, and are not completely removed in conventional wastewater treatment processes, leading to the need for in-depth studies for improvements in sewage treatment plants. Dipyrone, diclofenac and paracetamol are widely used drugs in the world as anti-inflammatories, analgesics and antipyretics. In Brazil, these medicines can be purchased at low cost and without prescription in pharmacies. The release of these emerging pollutants can lead to damage to human health and aquatic biota. The aim of this study was to present a literature review on the removal of dipyrone, diclofenac and paracetamol in wetlands sewage treatment systems. It has been observed that constructed wetlands can remove drugs as well as in conventional stations, such as ponds or activated sludge, due to the coexistence of different physicochemical conditions, with both aerobic and anaerobic degradation routes. The lack of in-depth studies comparing the removal of dipyrone, diclofenac and paracetamol in different treatment processes does not yet indicate the most efficient technology. Constructed wetlands are systems known to be efficient in the removal of carbonaceous and nitrogenous organic matter and can be applied for decentralized treatment. New research has shown that such systems can also remove drugs with high efficiencies in both horizontal and vertical hydraulic flow configurations in order to minimize the concentrations of these pollutants in water bodies. The possibility of applying natural sewage treatment systems that can remove biologically emergent pollutants makes WCs stand out in comparison to other technologies, especially in Brazil, where it has not yet been fully exploited in municipalities and small communities.

The fast development of laboratory methods has revealed increased amounts of trace concentrations of pharmaceuticals and personal care products (PPCP) in waste waters in the Czech Republic. This paper focuses on the expected costs to solve this problem by quaternary treatment of waste water based on activated carbon filtration. The one‐time investment costs in 155 wastewater treatment plants (WWTPs) with a capacity of over 10 000 population equivalent (PE) would represent an amount of around 300 million EUR. The increase in end‐user operating costs would be 0.4 EUR/m3, which would mean a 15% increase in water and sewage costs. A sociological survey showed that most respondents (65%) would agree with an increase in price but only by 10%. Currently the cost of the quaternary treatment of wastewater is based primarily on estimates. Therefore changes in legislation leading to stricter limits and an increase in the efficiency of wastewater treatment should be preceded by additional applied research.
