Occurrence of Psychosis in Patients with Epilepsy Randomized to Tiagabine or Placebo Treatment

Abstract: Summary:Purpose: Patients with drug-resistant epilepsy have a higher incidence of psychiatric problems and possibly greater intolerance to antiepileptic drugs (AEDs) than do other patients with epilepsy. Concern has been raised that ␥-aminobutyric acid (GABA)ergic drugs may be associated with treatment-emergent psychosis. Tiagabine (TGB; Gabitril), a new AED that blocks synaptic GABA uptake, was developed in trials of drug-resistant patients with epilepsy. We conducted ad hoc analyses of adverse events, drug i… Show more

“…Only a few controlled studies have focused on the comparison between AEDs and non-AEDs in this regard, except for several clinical trials with placebo-controlled procedures. [31][32][33] In the current study, we confirmed the psychogenetic potential of AED. Furthermore, SAEDs have a higher psychogenic potential than that of FAEDs.…”

“…Some authors have strongly advocated that any potent AED can precipitate psychosis precisely because of the potent anticonvulsant effects and that a subgroup of patients is particularly vulnerable to dramatic seizure reduction, irrespective of the AED used. In contrast, there are reports of psychotic symptoms specifically after exposure to certain AEDs [30,31,32,33] (including; ethosuximide [34], topiramate [35], felbamate [36], levetiracetam [37], zonisamide [38] ,vigabatrin [39], tiagabine [40] and lacosamide [41].…”

“…Placebo‐controlled studies of tiagabine showed that the risk of psychosis was not significantly increased (Sackellares et al 2002). However, the paradoxical provocation of de novo , non‐convulsive status epilepticus is a specific issue reported by different authors (Schapel and Chadwick 1996) and, in some selected cases, there may be a differential diagnosis with brief ictal psychotic episodes (Trimble 1991).…”
