Impact of Noise Exposure on Risk of Developing Stress-Related Metabolic Effects: A Systematic Review and Meta-Analysis

Background: Exposure to noise can increase biological stress reactions, which may increase adverse health effects, including metabolic disorders; however, the certainty in the association between exposure to noise and metabolic outcomes has not been widely explored. The objective of this review is to evaluate the evidence between noise exposures and metabolic effects. Materials and Methods: A systematic review of English and comparative studies available in PubMed, Cochrane Central, EMBASE, and CINAHL databases between January 1, 1980 and December 29, 2021 was performed. Risk of Bias of Nonrandomized Studies of Exposures was used to assess risk of bias of individual studies and certainty of the body of evidence for each outcome was assessed using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results: Fifty-six primary studies reporting on cortisol, cholesterol levels, waist circumference, glucose levels, and adrenaline and/or noradrenaline were identified. Although meta-analyses suggested that there may be an increase in waist circumference and adrenaline with increased noise exposure, the certainty in the evidence is very low. Overall, the certainty in the evidence of an effect of increased noise on all the outcomes were low to very low due to concerns with risk of bias, inconsistency across exposure sources, populations, and studies, and imprecision in the estimates of effects. Conclusions: The certainty of the evidence of increased noise on metabolic effects was low to very low, which likely reflects the inability to compare across the totality of the evidence for each outcome. The findings from this review may be used to inform policies involving noise reduction and mitigation strategies, and to direct further research in areas that currently have limited evidence available.

Introduction: Noise is an unwelcome sound that disrupts workers. Noise is present in every workplace, including ship engine noise. Continuous noise exposure can result in health issues, including hearing loss. Noise can cause stress on traditional boat workers because being continuously exposed to noise causes an uncomfortable feeling in the work environment. This uncomfortable feeling can trigger stress on ferry boat workers. This study aims to determine the relationship between noise and work stress on ferry boat workers at the Pier of Kampung Baru Tengah, Balikpapan. Methods: This study used a quantitative approach with a cross-sectional research design with 44 respondents. The instruments used a Sound Level Meter to measure Noise Level and Dass 21 Questionnaire with an interview method to measure Job Stress. Results: As many as 35 respondents were exposed to noise caused by traditional boat engines, and more than 50% of workers did not experience work stress (normal). The results of this study indicated a relationship between noise and work stress in traditional boat workers. Conclusion: The direction of the association between noise and work stress was positive but low, meaning that, as noise levels rise, so does the risk of workplace stress.

Introduction. The aim of this review was to investigate the possible association between noise exposure and cortisol hormone in all noise-exposed job classifications. Materials and methods. In this systematic review, the Medline (PubMed), Scopus, Web of Science, and Embase databases were searched from 1 January 1991 to 28 September 2021. As a result, all English relevant papers among any group of workers, in any workplace, and at any noise level were included. Abstracts without full texts, editorials, letters, case reports, conference proceedings, reviews, in-vitro, and animal studies were excluded. Furthermore, studies were removed in the case that they were conducted on children or general population, did not measure the noise levels in work environments, and simulated the workplace noise or used a noise chamber. Results. The initial search retrieved 5784 articles. Finally, 13 articles were considered for qualitative synthesis. Among all the included studies, most of them (10 vs. three), conducted in various industries reported a statistically significant association between occupational noise exposure and increased levels of cortisol hormone. Conclusion. Our qualitative synthesis of available evidence proposed a potential relationship between occupational noise exposure and cortisol hormone. So, noise can be considered an occupational health concern.
