Introduction: It is well known that ionizing radiation (IR) exposure increases the risk of thyroid cancer; however less is known about its role in development of autoimmune thyroiditis. Aim of work: to investigate the thyroid gland functions, level of anti TPO among interventional cardiology team members searching for an association between the level of their occupational exposure to ionizing radiation and the thyroid gland integrity. Materials and methods: This work was carried out in Cardiac Catheterization Unit in National Heart Institute Center. The study comprised two groups, an exposed group of 47 cardiac catheterization workers (19 physicians, 15 nurses and 13 technicians) and a control group of 47 workers matched with the exposed group as regards age and sex. The studied groups were subjected to a detailed questionnaire, full clinical examinations. laboratory investigations in the form of serum thyroid stimulating hormone (TSH), Triiodothyronine (T3), Thyroxine (T4) and antithyroxiperoxidase antibodies (anti-TPO). Radiation exposure was monitored by digital dosimeter. Results: Dosimeter readings were higher among physicians followed by nurses and technicians. Statistically significant higher level of serum TSH, anti-TPO along with statistically significant lower levels of T3, were found among the exposed compared to control groups (P<0.05). Significant positive correlation was detected between the level of anti-TPO and TSH (r= 0.876 P<0.001). Moreover, a significant negative correlation was found between anti-TPO and each of T3 (r= -0.814 P<0.001) and T4 (r= -0.324 P<0.05). ANOVA and Post Hoc tests showed significantly higher level of anti-TPO with significantly lower levels of T3 among the physicians compared to control group. Conclusion: Autoimmune thyroiditis may be considered as one of the adverse health hazards of occupational exposure to ionizing radiation in cardiac catheterization units. Recommendations: We recommend the regular measurement of thyroid hormone especially TSH and anti-TPO to detect early affection of thyroid gland among radiation exposed workers.

Objective: To investigate regional variations in the frequency of undiagnosed thyroid disorders among 25-to 88-year-old men and women in two communities in the northeast and the south of Germany. In addition, autoantibodies to thyroperoxidase (TPO-Abs) and urinary iodine excretion were determined. Methods: Two population-based surveys of men and women using a common standardized protocol, the Study of Health in Pomerania (SHIP) in the northeast of Germany (2505 participants) and the Kooperative Gesundheitsforschung in der Region Augsburg (KORA) in the south of Germany (2316 participants), were compared with regard to the frequency of undiagnosed thyroid disorders. Results: Compared with the northeast of Germany, urinary iodine excretion and serum thyroidstimulating hormone (TSH) levels were significantly higher in the south. The median urinary iodine concentration was 110 mg/l (64; 169 mg/l) in SHIP and 151 mg/l (97; 214 mg/l) in KORA, and the median TSH value was 0.81 mIU/l (0.56; 1.15 mIU/l) in SHIP and 1.22 mIU/l (0.84; 1.80 mIU/l) in KORA. The frequency of elevated TSH (TSH R2.12 mIU/l) was 4.3% in SHIP and 14.1% in KORA (P!0.001); the corresponding values for suppressed TSH (!0.25 mIU/l) were 3.5 and 1.7% (P!0.001). The proportion of ultrasonographic findings was 55.5% in SHIP and 68.0% in KORA. The frequency of serum TPO-Abs did not differ significantly between northeast and south Germany. Conclusions: There were considerable regional disparities in the frequency of thyroid disorders within Germany. These differences can be explained not only by different regional histories of natural iodine deficiency but also by current differences in the iodine supply under an identical nationwide iodine fortification program.

ĮvadasHašimoto tiroiditas, literatūroje dar vadinamas lėti-niu limfocitiniu ar autoimuniniu tiroiditu, pagal patogenezę ir klinikinę eigą yra susijęs su autoimunine hipertireoze -Greivso (Bazedovo) liga, dėl to abi šias skydliaukės patologijas galima vadinti autoimunine skydliaukės liga. Tiek humoralinis, tiek ląstelinis imunitetas turi įtakos autoimuninės skydliaukės ligos pasireiškimui. Diagnozuojant šią ligą, paciento serume randama antikūnų prieš fermentą -skydliaukės peroksidazę (mikrosominiai antikūnai), skydliaukės folikulų koloidą -tiroglobuliną (tiroglobulino antikūnai) ir prieš skydliaukę stimuliuojamąjį hormoną (angl. thyroid-stimulating hormone -TSH) receptorius (TSH receptorių antikūnai) (1). Sergant autoimunine skydliaukės liga, pačioje skydliaukėje sekretuojami skydliaukės autoantikūnai. Juos sintezuoja subrendusios B ląstelės (plazmocitai), sudarydamos skydliaukėje esančio limfocitinio infiltrato tam tikrą dalį. Skydliaukės svarbą, sekretuojant antikūnus, rodo tiroidito eksperimentiniai modeliai su gyvūnais, po atliktos strumektomijos, po gydymo tireostatikais ar radioaktyviuoju jodu taip pat nustatomas skydliaukės autoantikūnų titrų sumažėjimas (2).Ląstelinis imunitetas taip pat nukreiptas prieš tris pirminius skydliaukės antigenus, yra atsakingas už autoimuninės skydliaukės ligos pasireiškimą. Tai matyti iš T ląstelių gebėjimo sekretuoti limfokinus ar reaguoti mitogeniniu atsaku, kai šios ląstelės sąveikauja su skydliaukės antigenais ar su antigenų tam tikromis peptidinėmis sekomis. Ligoniai ir jų giminės dažniau serga kitomis autoimuninės kilmės endokrininėmis ir neendokrininėmis ligomis: pirmojo tipo cukriniu diabetu, piktybine mažakraujyste, sunkiąja miastenija,

Abstract: We conclude that occupational exposure to ionizing radiation is related to the risk of AITD. The usage of thyroid protection shields by radiation workers is strongly recommended.
