Occult breast abnormalities: percutaneous preoperative needle localization.

Abstract: The results of 500 preoperative radiographically guided localizations for occult breast lesions over a period of more than 5 years are described. Of the total, 117 (23.4%) were found to be malignant. Biopsy was prompted by abnormal calcifications in 41% (203/500) of the cases and by occult masses, areas of architectural distortion, or asymmetry in 59% (297/500). of 74 patients with carcinoma who underwent axillary lymph node sampling, 78% (58/74) were free of metastatic involvement.

“…Noncalcified masses represent 41â€"45% of nonpalpable breast lesions referred for biopsy [6,7]. In general, only 1â€"2% of circumscribed of the specimen was represented by lesion; the remainder of the specimen consisted of fat, blood, benign breasttissue,or skin.…”

“…In the United States, the positive predictive value (PPV) of mammography (the number of cancers diagnosed per number of biopsies recommended) has ranged from approximately 15% to 30% [2][3][4][5][6][7]; in the European trials, it ranged from 30% [8] to 75% [9]. Some of the reasons for such discrepancies have been discussed [1 0].…”

“…The overall biopsy positivity of 27% in our clinically heterogeneous group of women appears to be consistent with previous estimates. In other series,5'6 positive biopsy percentages ranged from 9% to 65%, with a 15% to 30% positive biopsy rate among asymptomatic women7- 16 and a 20% to 40% rate among symptomatic women.17-19 Our finding that fibrocystic disease accounted for 72% of all the benign diagnoses is similar to that of a previous study .20 It is noteworthy that the older women (who are known to be at higher risk for breast cancer) have lower rates of biopsy but a higher rate of positivity in our study. The With these data, we are able to measure the parameters of biopsy rate and biopsy positivity and how these parameters vary across subgroups of age.…”
