Occlusal Cant: Etiology, Evaluation, and Management

Abstract: Occlusal cant is an important factor affecting smile esthetics. The dynamic display zone includes lateral, vertical, and anteroposterior aspects, as well as the cant of the occlusal plane. Smile design and mechanotherapy must take into account an esthetic plane of occlusion. The purpose of this review is to describe occlusal cant, examine its diagnosis, and explain the treatment alternatives for this condition.

“…It is more prevalent in a patient with class-3 malocclusions. 1,2 The level of the occlusal plane is a prerequisite for the success of all orthognathic surgeries, and failure to level the canting of occlusal plane during surgery may have detrimental effects on masticatory functions. Recording of the occlusal cant at the initial stages is very important as the development of occlusal cant at a later stage in treatment usually points to faulty treatment biomechanics.…”

“…The occlusal plane is the plane established if a line was drawn through all the buccal cusp tips and incisal edges of the mandibular teeth and then Eur J Dent:2020;14:429-434 broadened into a plane to include the lingual cusp tips, continuing across the arch to include the buccal and lingual cusp tips of the opposite side. 5 Soft tissue becomes one of the factors causing Class II malocclusions, such as in the case of Class II division 1, causing a hypotonic upper lip or retrusion of lower jaw incisors due to a hyperactive lower lip. 6 Mandibular rotation is closely related to occlusal plane changes during growth.…”
