Obturator hernia: The plug technique

Abstract: The obturator hernia is an uncommon condition, with clinical manifestations of pain and intestinal obstruction. The preoperative diagnosis is difficult. The treatment is always surgical. There are several repair techniques that have been described: sac ligation alone, direct suture repair, use of autologous tissue or prosthetic repair. We report a case of an obturator hernia that was treated by the use of a plug of Mersilene.

“…Over the years, various methods have been applied and modified to treat this kind of disease, ranging from the simple surgery of fascia stapling, to laparoscopic techniques and modern materials, such as synthetic meshes [1]. The application of alloplastic implants has decreased the risk of the recurrence of the disease, as well as the intensity of postoperative pain [2].…”

“…In more than 80% of cases, patients present with symptoms of small bowel obstruction caused by incarceration of the bowel into the obturator canal [2]. The classic presenting sign is the Howship-Romberg sign, which occurs in 48% of cases, characterized by pain or paresthesia in the hip with radiation down the medial thigh to the knee on the affected side.…”

“…Shapiro et al [6] describe the use of polypropylene mesh placed extraperitoneally to remedy the hernia. Insua et al [2] used a polyester plug that was inserted into the obturator canal to prevent incarceration in the future. Although the plug technique may prevent future incarceration, it would seem that such a repair would not alleviate nerve compression through the obturator canal.…”

“…When last seen, this patient had no complaints about the operation. A plug repair is now often performed via the inguinal or preperitoneal approach, or even in laparotomy or laparoscopy, 13,14 and severe complications related to the plug insertion have seldom been reported.…”
