Objective: To know the characteristics of patients with obstetrics and gynecologic fistula in Dr. Cipto Mangunkusumo Hospital, Jakarta. Method: This study was a descriptive design using secondary data from medical records and patient database during 2011-2016. Result: There were 68 subjects with fistula. From 2011-2016, there were 5 cases (7.4%), 19 cases (27.9%), 16 cases (23.5%), 11 cases (16.2%), 8 cases (11.8%), and 9 cases (13.2%) of fistula; respectively. The average age of subjects was 38 years old and only 34 subjects have complete data. Gynecologic and obstetric fistula were 17 cases each. As total 28% of fistula cases were vesicovagina fistula, 12% were rectovaginal fistula, 9% were anovagina fistula, and the other types of fistula were 2%. Among 34 subjects, there were 21% of vesicovagina fistula and 3% of rectovagina fistula, which were caused by iatrogenic. There were 7% of vesicovagina and rectovagina fistula each and 9% of anovagina fistula were caused by obstetric problems. The iatrogenic procedures found from this study were total abdominal hysterectomy (16%), vaginal hysterectomy (3%), and neovagina (3%). Conclusion: The trend of cases is decreasing during the present years (2011-2016). Gynecologic fistula cases caused by iatrogenic are the major problems, but the obstetrics fistula cases decreasing following the labor monitoring is much better nowadays. Background: Globally, prolonged and obstructed labor contributed to 8% of maternal deaths which can be reduced by proper utilization of partograph during labor. Methods: An Institution based cross-sectional study was conducted in June, 2013 on 403 obstetric care providers. A pre-tested and structured questionnaire was used to collect data. Data was entered to EpiInfo version 3.5.1 statistical package and exported to SPSS version 20.0 for further analysis. Logistic regression analyses were used to see the association of different variables. Results: Out of 403 obstetric care providers, 40.2% utilized partograph during labor.Those who were midwives by profession were about 8 times more likely to have a consistent utilization of the partograph than general practitioners (AOR=8. 13, 95% CI: 2.67, 24.78). Similarly, getting on job training (AOR=2. 86, 95% CI: 1.69, 4.86), being knowledgeable on partograph (AOR=3. 79, 95% CI: 2.05, 7.03) and having favorable attitude towards partograph (AOR=2. 35, 95% CI: 1.14, 4.87) were positively associated with partograph utilization. Conclusion: Partograph utilization in labor monitoring was found to be low. Being a midwife by profession, on job training, knowledge and attitude of obstetric care providers were factors affecting partograph utilization. Providing on job training for providers would improve partograph utilization.

Background: To date, obstetric fistula is one of the major public health challenges among the poorest, young women and girls in rural communities in low and middle-income countries. There is scarce information on the association of socio-demographic and obstetric factors with the obstetric fistula patients’ perceptions towards life fulfillment which compelled this study.Methods: A cross-sectional survey using a semi-structured questionnaire was done among 390 obstetric fistula patients registered within the past two years to the time of data collection at Kitovu Hospital, Masaka, Uganda. Pearson’s Chi square correlation was used to determine the association of socio-demographic and obstetric factors with perceptions towards life fulfillment using SPSS 25.0.Results: A larger proportion of obstetric fistula patients 226 (57.9%) had negative perceptions while 164 (42.1%) had positive perceptions. Maternal age, repair status, outcome of repair, self-perceived level of incontinence, and number of living children were statistically associated with patients’ perceptions towards life fulfillment at a P-value less than 0.001, marital status, p value of 0.005, and overall duration lived with fistula, p value of 0.009. A greater number of patients with tertiary education, 5 (83.3%) had positive perception, however, the association was not significant with a p value of 0.059. Fistula type, region of residence, and number of repairs were not statistically significant.Conclusions: Thus, socio-demographic and obstetric factors influence patients’ perceptions towards life a great deal. Efforts should focus on fistula prevention and timely repair so as to minimize probable experiences likely to result in negative perception that would eventually impede life fulfillment.
