Obsessive-Compulsive and Related Symptoms in Children and Adolescents With Rheumatic Fever With and Without Chorea: A Prospective 6-Month Study

Abstract: The association between Sydenham's chorea and OCD supports suggestions that similar mechanisms involving the basal ganglia underlie both disorders. Obsessive-compulsive symptoms occurred at the beginning of rheumatic fever, so early psychopathological assessments are essential.

“…138,139 Streptococcal infection, RF and the impact on future studies Based on reports of a higher prevalence of OCD in patients with Sydenham's chorea (SC, the neurological manifestation of RF), a number of studies were undertaken and the results suggest a possible relationship between streptococcus infection and OCD. 140 Additional studies reported a higher prevalence of OCD and TS in RF, even without SC, both in active 141,142 and nonactive 143 phases. Furthermore, familial aggregation of OCD/TS has been described in relatives of RF cases (Hounie et al, personal communication).…”

“…7 In Brazil, in a study of 30 patients with chorea, OCD and/or tics were observed in 70% of cases. 10 Studies comparing children with OCD and/or tics and a healthy control group revealed that the former were more closely associated with streptococcal infections, suggesting that the symptoms might result from infectious autoimmune phenomena.…”

“…Relatos de casos vêm sugerindo, desde o século XIX, uma relação ente coréia de Sydenham, a manifestação da febre reumática no SNC, obsessões e tiques. Mais recentemente, estudos sistematizados verificaram prevalências aumentadas de SOC e TOC em crianças com coréia de Sydenham [54][55][56] .…”
