Observations on the efficacy of propranolol for the prophylaxis of migraine

Abstract: Among 1,036 patients treated for migraine headache between 1973 and 1980, 865 were given a daily maximum dose of 320 mg of propranolol, while 171 served as differently treated controls. At least one year of treatment was prescribed, at the end of which time 84% of the propranolol group reported sustained improvement compared with similar improvement in only 32% of controls (p less than 0.01). The success rate progressively increased during the entire year.

“…Propranolol hydrochloride is used routinely in adults at doses of 240-320mg/day for indications such as migraine prophylaxis 15,16 and essential tremor 17-19 and has been used safely in burn patients to decrease the metabolic consequences of burn injury. 20-22 Based on this safety profile and lack of established dose regimen for burn pain, a dose of 120mg BID of propranolol (240mg/day) was selected for patients receiving active study drug.…”

“…As with other β-blockers lacking partial agonist activity, atenolol is highly effective in the prophylaxis of migraine, its prophylactic effect being time-and dose-related. 17,18 Despite some concern on the use of (β-blockers in complicated migraine, hemiplegic migraine, and migraine with aura, due to a potential risk of stroke, 19 this complication has not been seen in many trials. 17 The patient reported here showed a clear change in the natural history of her migrainous syndrome after atenolol was started.…”

“…As in migraine unrelated to brain lesions, her pattern of response was time-and dose-related. 18 Initial delay in diagnosis was due to difficulties in the health care system of our country, but cerebral angiography was postponed by the patient and her family either because of denial of the severity or by the almost complete response to atenolol prophylaxis. In another unpublished case followed by one of the authors, it was also the progressive worsening of the migrainous syndrome that lead the patient to seek specialized medical care.…”
