Observational studies: why are they so important?

Abstract: Randomized clinical trials (RCTs) are known to be the gold standard for medical research, and this study design is preferred for investigating the efficacy of new interventions. Many authors and editors believe that the RCT design always surpasses other research designs. However, this is not an unquestionable truth. The main advantage of RCTs is that they provide better control over possible bias through randomization and blinding. In other words, the great strength of an RCT is its high internal validity. On t… Show more

“…Nevertheless, when RCTs are not feasible, a well-designed observational study may be key. [33] The observational method allows for direct observation of a clinical tool without manipulation. [34] An observational study without randomization would be most appropriate as we aim to better understand how best to mitigate the impact of SARS-CoV-2 on lung pathophysiology.…”

“…In this way, observational trials can be complementary to RCTs in spite of their evidence level being lower than that of RCTs. This kind of investigation is crucial for elucidating many scientific questions [ 31 ]. Observational trials cannot replace RCTs nor do RCTs make non-experimental studies unnecessary or undesirable.…”

“…Similarly, there are also Retrospective Cohort Studies (RCSs) for treatment effect estimation [ 15 ], in which the data of the treatment and control group are selected from historical data based on inclusion criteria, but RCSs still have the identical baseline distributions in the two groups to avoid selection bias. However, RCTs may not be feasible in many cases, e.g., forcing non-smokers to smoke in an RCT for smoking is against ethics [ 16 ]. Even in the cases where RCSs are feasible, the strict inclusion criteria limits the generalizability of RCTs for the patients who cannot be represented by the included ones [ 17 , 18 ], which is also the case for RCSs.…”
