Observation of Gastric Variceal Flow Characteristics by Endoscopic Ultrasonography Using Color Doppler

Abstract: ECDUS is a useful modality for diagnosis of the hemodynamics of gastric varices and may allow the prediction of a high risk for hemorrhage.

“…We previously reported use of ECDUS for evaluating the hemodynamics of esophago-gastric varices [11,12]. Iwase et al [13] reported that ECDUS is a useful method for the evaluation of gastric varices and for endoscopic obliteration of varices with cyanoacrylate glue.…”

“…In addition, fast Fourier transform analysis can be used to evaluate flow patterns and to calculate the velocity of blood flow at gastric varices [4][5][6]. Based on their locations, ordinary gastric varices can be classified as either fundal, or cardiac and fundal (located between the cardiac orifice and the fundus); however, in no cases of ordinary gastric varices examined using ECDUS did varices show expansion to the curvatura ventriculi major of the gastric body [4].…”

“…Additional images resulting from ECDUS color analysis of gastric variceal flow clearly depicted a round fundal region at the center, with varices that expanded to the curvatura ventriculi major of the gastric body on gastric varices due to splenic vein occlusion (Figures 3a and 3b) [23]. Variceal location of ordinary gastric varices was classified as fundal (located far from the cardiac orifice) and cardiac and fundal (located between the cardiac orifice and the fundus), however, there was no case with varices expanding to the curvatura ventriculi major of the gastric body with ECDUS findings of ordinary gastric varices [30]. These data provide specific findings that can function as hallmarks of gastric varices due to splenic vein occlusion.…”

“…In a previous report, Sato et al described the utility of endoscopic color Doppler ultrasonography (ECDUS) in patients with gastric varices [30]. Relative to EUS, ECDUS color images of blood flow in vessels allow for detailed sonographic visualization of vessels and evaluation of vascular blood flow for the diagnosis of gastric varices [31,32].…”
