Background: Obesity is a well-known risk factor for many health problems including endometrial cancer. In addition, it may act as an obstacle to achieving optimal treatment of endometrial cancer and may be associated with disease recurrence. Aim: To explore the impact of obesity on the operative procedure, recurrence of disease and survival in patients with endometrial cancer. Methods: A retrospective study of 267 patients with endometrial carcinoma who underwent surgery at the Oncology Center-Mansoura University from January 2011 to December 2017. Patients were divided according to their body mass index (BMI) into two groups, Group 1 with a BMI <30 and Group 2 with a BMI ≥30. Results: Group 1 included 46 patients and Group 2 included 221. The mean operative time was longer in Group 2 without statistically significant difference. Twenty-two (47.8%) patients in Group 1 and 71 (32.1%) in Group 2 underwent lymphadenectomy (p=0.062). The estimated blood loss was significantly higher in Group 2 (p<0.05). No statistically significant difference was found between the two groups regarding the intra or post-operative complications, despite the high incidence of complications in Group 2. The median disease-free survival (DFS) was 74 months in Group 1 vs. 66 months in Group 2. Obesity did not have a statistically significant impact on DFS among the studied cases (p=0.24). Conclusion:In the current study, obesity did not significantly impact the operative procedure, surgical complications or DFS in patient with endometrial cancer.

Endometrial cancer (EC) is the most frequently observed malignant gynecologic disease in developed countries. There is a strong association between the established risk factor obesity and the incidence of EC. Furthermore, the rate of women with a body mass index (BMI) > 30 kg/m2 is increasing worldwide, correspondingly leading to a higher prevalence of EC. Understanding the adipose tissue as an endocrine organ, elementary pathophysiological pathways of tumorigenesis have been revealed. This includes the fundamental role of hyperglycemia, insulin resistance, and hyperestrogenemia, as well as interactions with a chronic proinflammatory microenvironment. Therapeutic options potentially include metformin or bariatric surgery. Moreover, changes in individual lifestyle such as weight reduction, physical activity, and an awareness of healthy nutrition are effective in preventing the disease.

Mengetahui profil penderita kanker endometrium. Metode: Penelitian metode deskriptif dengan menggunakan data sekunder. Kriteria inklusi penelitian yaitu pasien dengan diagnosis kanker endometrium dilihat berdasarkan hasil histopatologi. Hasil: Distribusi pasien mayoritas berusia >50 tahun (62,0%), berasal dari Kota/Kabupaten Bandung (32,5%), memiliki indeks massa tubuh 18,5-22,9 kg/m 2 (27,0%), multipara (36,5%), status postmenopause (59,0%), memiliki usia menarche ≥12 tahun (88,0%), tidak memiliki riwayat diabetes mellitus (66,0%) dan hipertensi (27,0%), terdiagnosis ketika stadium I (47,5%), dengan derajat diferensiasi baik/grade I (31,0%) dan tipe I endometrioid adenocarcinoma (82,5%). Terapi yang paling sering adalah tindakan operasi (50,0%) dengan tipe pembedahan histerektomi total dan salfingooforektomi bilateral (44,21%). Kesimpulan: Pasien kanker endometrium di RSUP Dr. Hasan Sadikin Bandung periode tahun 2017-2020 ditemukan paling banyak berusia >50 tahun, dari Kota/Kabupaten Bandung, indeks massa tubuh normal, multipara, postmenopause, usia menarche ≥12 tahun, tidak ada riwayat DM dan hipertensi, terdiagnosis pada stadium I dengan tipe I endometrioid adenocarcinoma dan derajat diferensiasi baik (grade I), serta dilakukan tindakan operasi dengan tipe pembedahan histerektomi total dan salfingooforektomi bilateral.
