Malnutrition is common among residents of homes for the elderly. This study aimed to identify the nutritional status of people in residential homes for the elderly in Lattakia, Syrian Arab Republic, and to determine the factors that affected nutritional status in these homes. A total of 103 elderly people in 3 residential homes were interviewed individually using an Arabic version of the Mini Nutritional Assessment (MNA) scale, and anthropometric measurements were carried out to assess nutritional status. The mean age was 70.9 (SD 6.4) years. Two-thirds of residents were either at risk of malnutrition (39.8%; score 17-23 on the MNA) or malnourished (19.4%; score < 17 on the MNA). Mean body mass index was 22.0 (SD 4.7) kg/m 2 . Nutritional status was significantly affected by age, level of education, source of income, duration of stay in the home, number of diseases, number of medicines taken, anthropometric data and teeth and vision problems. RÉSUMÉ La malnutrition est fréquente chez les résidents des maisons de retraite. La présente étude visait à déterminer l'état nutritionnel des résidents des établissements pour personnes âgées à Lattaquié (République arabe syrienne) ainsi que les facteurs qui influent sur cet état nutritionnel dans ces résidences. Au total, 103 personnes âgées dans trois maisons de retraite ont été interrogées en entretien individuel en recourant à la version en langue arabe de l'échelle d'évaluation de l'état nutritionnel Mini Nutritional Assessment (MNA), et des mesures anthropométriques ont été réalisées pour évaluer leur état nutritionnel. L'âge moyen était de 70,9 ans (ET 6,4) . Les deux tiers des résidents étaient soit à risque de malnutrition (39,8 % ; score 17-23 sur l'échelle MNA), soit malnutris (19,4 % ; score < 17 sur l'échelle MNA). L'indice de masse corporelle moyen s'élevait à 22,0 kg/m 2 (ET 4,7). A 73-year-old woman presents to your clinic complaining of unintentional weight loss. She reports having lost 15 lbs (6.8 kg) over the past year. Previously, her weight was 135 lbs (61.3 kg) and now it is 120 lbs (54.5 kg). She reports that she is eating three meals per day as usual.The patient's past medical history is notable for osteoporosis, a left hip fracture three years ago, osteoarthritis, hypothyroidism and hypercholesterolemia. She is taking levothyroxine, a statin and a bisphosphonate plus vitamin D and calcium. She recently started taking nonsteroidal anti-inflammatory medications (NSAIDs) as needed for knee pain. She is an exsmoker and does not drink alcohol. On examin ation, her weight was 121 lbs (55 kg), with a body mass index (BMI) of 22. She also had osteoarthritic changes in the knees. Results of investigations, including radiograph of the chest, complete blood count, electrolytes, creatinine, thyroid stimulating hormone and albumin are within normal limits.

Este estudo objetivou avaliar a concordância interobservadores de medidas antropométricas e avaliação subjetiva do estado nutricional em adultos hospitalizados. Pacientes internados em um hospital geral universitário tiveram peso atual, peso usual, altura, circunferência do braço, prega cutânea tricipital, percentual de perda de peso, Índice de Massa Corporal, circunferência muscular do braço e Avaliação Nutricional Subjetiva Global obtidos por avaliadores independentes. A concordância foi avaliada pelo coeficiente kappa, teste t pareado e coeficiente de correlação intraclasse (ICC). Foram avaliados 102 pacientes, com idade de 53±15 anos. Foi obtida boa correlação entre observadores para todos os métodos antropométricos (ICC>0,86) e diferenças médias de pequena magnitude. As variáveis categorizadas mostraram concordância ótima para o Índice de Massa Corporal (kappa=0,98; IC95%: 0,95-1,0), fraca para a Avaliação Nutricional Subjetiva Global (kappa=0,46; IC95%: 0,31-0,60) e moderada para os percentis 5 e 10 da circunferência do braço (kappa=0,78; IC95%: 0,65-0,91), circunferência muscular do braço (kappa=0,73; IC95%: 0,54-0,92) e prega cutânea tricipital (kappa=0,65; IC95%: 0,48-0,82). Para muitos pacientes houve discrepâncias clinicamente relevantes no percentual de perda de peso, circunferência do braço, prega cutânea tricipital e circunferência muscular do braço. Embora estatisticamente a reprodutibilidade dos métodos antropométricos tenha sido boa, deve-se ser prudente ao empregá-los no contexto hospitalar, visto as diferenças encontradas, em especial na antropometria do braço e na Avaliação Nutricional Subjetiva Global.
