Background: Approximately 20-30% of obese patients do not achieve successful weight outcomes after bariatric surgery. Objective: We examined whether short-term changes (#0.5 y postsurgery) in energy intake and macronutrient composition after bariatric surgery could predict 10-y weight change. Design: Participants were recruited from the Swedish Obese Subjects (SOS) study, which was a matched (nonrandomized) prospective trial that compared bariatric surgery with usual care for obese patients. A total of 2010 patients who underwent bariatric surgery were included in the study. Physical examinations (e.g., weight) and questionnaires (e.g., dietary questionnaire) were completed before and 0.5, 1, 2, 3, 4, 6, 8, and 10 y after surgery. For the main analytic strategy, a linear mixed model was implemented, which included repeated measures with a random intercept and an unstructured covariance matrix. Results: Short-term changes in energy intake (P , 0.001) and in relative proportions of energy from carbohydrates (P , 0.001), fat (P , 0.001), and protein (P , 0.05) were associated with 10-y weight change after bariatric surgery. At the 10-y follow-up, men and women with the largest reductions in energy intake had lost 7.3% and 3.9% more weight, respectively, compared with that of subjects with the smallest intake reductions (P , 0.001). Greater weight loss was achieved in men and women who favored protein and carbohydrates over fat and in subjects who favored protein over carbohydrates than in individuals who favored the opposite changes in macronutrient composition (P , 0.05). Conclusions: The level of energy restriction that is achieved at 0.5 y after bariatric surgery predicts long-term weight loss. Weight loss is also associated with a changing dietary macronutrient composition. This trial was registered at clinicaltrials.gov as NCT01479452.Am J Clin Nutr 2017;106:136-45.

Introducción: El objetivo de este trabajo fue determinar la proporción de pacientes que cumplen criterios internacionales de gastrectomía en manga, así como estimar la asociación entre porcentaje de pérdida de peso con tasa de abandono del tratamiento nutricional. Material y Métodos:Cohorte retrospectiva realizada con historias clínicas de pacientes con gastrectomía en manga entre 2008-2014 en una clínica privada. Hubo dos variables resultado de interés: los criterios de inclusión para cirugía bariátrica según criterios internacionales (IMC≥40kg/m 2 o ≥35kg/m 2 con comorbilidades) y el abandono antes o durante (inasistencia ≥60 días desde la consulta previa sin recibir alta [IMC<25]) del tratamiento nutricional. La exposición fue el porcentaje de peso perdido en las primeras cinco semanas después de la cirugía (<10% vs. ≥10%). Se utilizaron modelos de regresión de Poisson. Se calcularon riesgos relativos e intervalos de confianza al 95% (IC95%).Resultados: La muestra incluyó 423 historias clínicas, 294 (69,5%) mujeres, edad media 39,2 (±12,3) años. 117 (27,7%) cumplieron requisitos para cirugía y 48 de esos 117 (41%) abandonaron el tratamiento nutricional antes de iniciarlo. Los pacientes que perdieron ≥10% de peso en el primer mes tuvieron un 90% menos de riesgo de abandonar la terapia nutricional que los que perdieron <10% del peso (RR=0,10; IC95%:0,01-0,68), ajustando por sexo y edad.Conclusiones: La mayoría de pacientes bariátricos no cumple los requisitos internacionales. Una mayor pérdida de peso en el primer mes se asoció a un menor abandono del tratamiento nutricional. The development of these updated guidelines was commissioned by the AACE, TOS, and ASMBS Board of Directors and adheres to the AACE 2010 protocol for standardized production of clinical practice guidelines (CPG). Each recommendation was re-evaluated and updated based on the evidence and subjective factors per protocol. Examples of expanded topics in this update include: the roles of sleeve gastrectomy, bariatric surgery in patients with type-2 diabetes, bariatric surgery for patients with mild obesity, copper deficiency, informed consent, and behavioral issues. There are 74 recommendations (of which 56 are revised and 2 are new) in this 2013 update, compared with 164 original recommendations in 2008. There are 403 citations, of which 33 (8.2%) are EL 1, 131 (32.5%) are EL 2, 170 (42.2%) are EL 3, and 69 (17.1%) are EL 4. There is a relatively high proportion (40.4%) of strong (EL 1 and 2) studies, compared with only 16.5% in the 2008 AACE-TOS-ASMBS CPG. These updated guidelines reflect recent additions to the evidence base. Bariatric surgery remains a safe and effective intervention for select patients with obesity. A team approach to perioperative care is mandatory with special attention to nutritional and metabolic issues.

Nutritional Management of Weight Regain After Bariatric Surgery

Abstract: Despite the short period of time, we observed that the nutritional counseling reduced the weight of patients with previous weight regain. There was also a reduction in body fat, which improves the perspective of weight maintenance in the future.
