Aim. This study aims to investigate the prevalence of disability, factors influencing disability and pain self‐management techniques employed by older arthritis patients in Taiwan. Background. Arthritis is the most common chronic disease in older people. It may result in pain, stiffness and joint deformity, which ultimately lead to disability. Understanding the factors influencing disability is needed to help arthritis sufferers to achieve optimal health status. Design. A cross‐sectional design was employed for this study. Methods. One hundred and fifty‐one older persons diagnosed with either rheumatoid arthritis or osteoarthritis were interviewed in the rheumatology clinics located in two medical centres in northern Taiwan. Six questionnaires were administered: Barthel Index, Instrumental Activity of Daily Living, Rapid Assessment of Disease Activity in Rheumatology, the Chinese version of Pain Management Inventory, Geriatric Depression Scale and Life Satisfaction Index. Results. Disability was found in 11% of Taiwanese individuals diagnosed with either rheumatoid arthritis or osteoarthritis. Those in disability reported more severe disease activity, pain, depression and lower life satisfaction. Hierarchical multiple regression analysis revealed that 31–46% of the total variance of disability could be explained by age, gender, marriage, joint pain score, diagnosis, disease activity, depression and pain management. Patients with rheumatoid arthritis had significantly higher levels of disability, disease activity during the preceding six months, more depression and less life satisfaction than patients with osteoarthritis. Conclusion. Higher disability was explained by older age, female, unmarried, diagnosed with rheumatoid arthritis, more joint pain, more disease severity, more depression and more use of pain management strategies in arthritis patients. Relevance to clinical practice. Nurses are urged to recognise the individual differences among the factors that are thought to contribute most to disability. An individualised, multidimensional and comprehensive treatment plan with informational support is essential to maximise pain management skills of arthritic older people to achieve improvement in pain, level of disability and mental health.

Aim: To assess the frequency at which various chronic pain care (CPC) approaches were used while managing older residents of the Health Service Facilities for the Elderly Requiring Care (HSFERC) in Japan and to assess the factors related to nurses and care workers that influence this care. Methods: A descriptive study design was used. The population comprised 31 nurses, 92 care workers, and 18 residents with chronic pain in eight HSFERC centers located in three provincial cities in Japan. A questionnaire was formulated by using the data collected by a literature review to assess the frequencies at which various CPC approaches were applied and the factors that might influence this care. Results: The most frequently preferred CPC approaches were gentle handling and support while providing daily care, listening attentively, and providing a recreational activity. The factors that affected the provision of CPC were the qualifications, years of experience of aged care, and experience of studying about chronic pain. The nurses tended to have a misconception regarding the manner in which the residents complained of pain and their pain sensitivity. Furthermore, organizational strategies for pain management were not reported by the nurses and care workers. Conclusions: In order to provide effective and active CPC, ongoing education about pain and cooperation between nurses and care workers to manage residents' pain are highly recommended.

Background: Pain is a symptom that affects quality of life negatively for many people. As a person gets older, the symptoms will increase and more than 50 % of the elderly in the municipal health care is expected to have some sort of pain. This makes pain management a key question. Aim: The aim of this study was to describe what nurses perceive affect pain management for older people. Method: The study is based on interviews with 18 nurses from two municipalities in Sweden. The method used was qualitative content analysis. Results: Four categories affecting pain management were revealed: Proficiency, communication, organization and treatment strategies. Within each category three to four sub-categories were formed, such as medicine and older, documentation, work environment and pharmacological treatment. Several variations were revealed within each sub-categories. The results showed possibilities for good pain relief but the lack of tailored pain assessment tools, communication and treatment strategies tailored for older patients in pain made it difficult. The emerging theme was an unstructured organization makes it difficult to provide good pain relief Conclusion: If evidence-based pain management strategies including pain assessment tools adapted in municipal health were developed, more patients should receive adequate pain treatment KEYWORDS: Nursing care, older adults, oldest old, pain assessment and pain management.
