Background: Extracorporeal membrane oxygenation (ECMO) therapy is proven to be efficient in a selected critically ill patient population and is showing promising results in refractory hypoxemia in the novel coronavirus disease (COVID-19). However, it necessitates specialized clinicians and advanced technological resources. While COVID-19 is an ongoing global emergency, there is no evidence-based practice in preparedness. This article proposes an innovative and optimized nursing care protocol, the Standard Operating Procedure (SOP), that increase safety, and prevents overuse of personal protective equipment (PPE) without interfering with therapy during ECMO in COVID-19 patients. Methods: A high-fidelity simulation scenario was developed. It included practicing safe and proper PPE donning and doffing during work organization, ECMO cannulation and ECMO-related procedures, and routine daily nursing care and management of patients on ECMO over nine hours. Results: Results were collected in three parts – proposal of nursing standardized operating procedures. They provide including work organization, work load references, competences and infrastructural conditions. Additionally, the cannulation equipment checklist proposal and daily routine nursing algorithm with other procedures during extracorporeal support were created.Conclusions: High-fidelity medical simulation can play an important role in staff training, improvement in previously gained proficiency, and development of optimal SOP for nursing care and management during ECMO in patients with COVID-19. Optimal SOPs may further guide multidisciplinary teams including intensive care units and interventional departments.

SARS-CoV-2 Leakage From the Gas Outlet Port During Extracorporeal Membrane Oxygenation for COVID-19

Patients with the coronavirus disease 2019 (COVID-19) sometimes develop refractory respiratory failure and may require venovenous extracorporeal membrane oxygenation (VV-ECMO). It is known that the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is sometimes present in the blood of COVID-19 patients. VV-ECMO is often used for several weeks, and plasma leaks can occur, albeit rarely. Hence, in terms of infection control, a concern is that SARS-CoV-2 may leak from the gas outlet port of the oxygenator during ECMO support of critically ill COVID-19 patients. The aim of this study was to clarify whether SARS-CoV-2 leaks from the oxygenator during ECMO support. Five patients with critical COVID-19 pneumonia were placed on VV-ECMO. Silicone-coated polypropylene membrane oxygenators were used in the ECMO circuits for these patients. SARS-CoV-2 ribonucleic acid (RNA) was measured by quantitative reverse transcription polymerase chain reaction in serum and at the gas outlet port of the ECMO circuit at the time of circuit replacement or liberation from ECMO. SARS-CoV-2 RNA was detected in the gas outlet port of the ECMO circuit for three of the five patients. None of the medical staff involved in the care of these five patients has been infected with COVID-19. In conclusion, SARS-CoV-2 could leak to the gas outlet port of the ECMO circuit through silicone-coated polypropylene membranes during ECMO support of critically ill COVID-19 patients.

Development and validation of the novel nursing activities in the care of extracorporeal membrane oxygenation‐supported patients scale

BackgroundNurses are members of multidisciplinary teams providing care to extracorporeal membrane oxygenation‐supported patients (ECMO‐SP). There are no valid and reliable measurements available to assess nursing activities in the care of ECMO‐SP.AimTo develop and evaluate psychometric properties of a new 27‐item nursing activities in the care of ECMO‐SP scale.Study DesignA convenience sample was recruited of 76 registered nurses (mean age 41.3 ± 8.7; 71% female) in Israel. The nurses participated in an online survey with software Qualtrics, between March and June 2022. Content and construct validity, as well as internal consistency of the scale were evaluated. The participants also completed the perceived knowledge and skills in the treatment of ECMO‐SP scale. The correlations between nursing activities in the care of ECMO‐SP and ECMO knowledge were examined using Pearson's correlation.ResultsExploratory factor analysis extracted four factors, and seven items were excluded from the 27‐item scale making it a 20‐item scale in its final form. Internal consistency was acceptable for the total scale (α = 0.84) and the four subscales: Factor 1: Nursing care of ECMO‐SP not related to the ECMO device (α = 0.90), Factor 2: Activities on ECMO device in emergency (α = 0.82), Factor 3: ECMO device calibrating and monitoring (α = 0.72), and Factor 4: Medication and blood administration through ECMO device (α = 0.73). When examined in confirmatory factor analysis, the 20‐item scale yielded acceptable fit indices. A positive correlation was found between perceived ECMO knowledge and all four factors of the nursing activities in the Care of ECMO‐SP scale.ConclusionThe 20‐item scale examining nursing activities in the care of ECMO‐SP is ready for use for clinical, educational, and research purposes.Relevance to Clinical PracticeBecause there are still no standards for nursing care of ECMO‐SP and no agreed quality indicators, this novel scale may help in building such standards and quality indicators.
