A atuação do time do Serviço de Atendimento Móvel de Urgência (SAMU) bem como o time de resposta rápida (TRR) reduz a letalidade relacionada à parada cardiorrespiratória (PCR). Contudo, é importante analisar, de forma mais aprofundada este quadro clínico, para entender melhor os desfechos clínicos associados. O presente estudo objetiva verificar o desfecho de pacientes em parada cardiorrespiratória atendidos pelo SAMU e TRR, segundo a literatura nacional. Este trabalho foi desenvolvido a partir de revisão sistemática da literatura realizado nos bancos de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e na biblioteca eletrônica Scientific Eletronic Library On-line (SciELO) com pesquisa de artigos, revistas, jornais, periódicos em língua portuguesa e inglesa, entre os anos de 2011 e 2023 usando os descritores: Parada Cardiorrespiratória, Serviço de Atendimento Móvel de Urgência, Enfermagem em Emergência, Risco ou desfecho a fim de compreender a atuação do SAMU com pacientes com Parada Cardiorrespiratória. Foram considerados apenas artigos que, após leitura do resumo, estavam de acordo com o tema do presente estudo, os demais foram desconsiderados. Foi identificado que a PCR é a emergência clínica mais grave, que pode gerar dano celular grave. Sendo assim, quanto mais rápida for sua identificação e aplicação de Ressussitação Cardiopulmonar (RCP), mnemônico ABCD e manobras rápidas de desfibrilação, melhor é o prognóstico, sendo estas as atividades mais utilizadas pelo SAMU. O PCR possui alta taxa de morbimortalidade e ainda assim muitos profissionais possuem deficiência sobre as novas diretrizes da American Heart Association (AHA) o que reflete no atendimento dos pacientes. Foi possível identificar lacunas no conhecimento sobre o protocolo que se aplica a PCR por parte do profissional enfermeiro, o que sugere inclusão deste tema com maior profundidade durante a graduação. A PCR pode ocorrer em diversas situações e lugares onde podemos compreender o papel vital da participação dos profissionais do SAMU para fornecer socorro imediato à vítima, no ambiente extra-hospitalar.

Introduction: The rapid response team is formed in hospitals with the aim of promoting the reduction in the occurrence of adverse events and reducing the risk and damage, disability or death of patients admitted to wards. These teams work to act quickly and early in situations of clinical deterioration in hospital wards, generally indicated by the use of early warning scales, an example being the MEWS score. The implementation of these teams, however, faces challenges and the extent of this problem in our country is not known for sure. Method: This study sought to identify and analyze scientific evidence on the challenges of implementing the rapid response team, according to the available literature. An integrative review was carried out based on the guiding question 'what is the evidence on the challenges of implementing the rapid response team, according to the scientific literature' in the databases Medical Literature Analysis and Retrievel System Online (MEDLINE), Latin American Literature and Caribbean Health Sciences (LILACS) and Nursing Database (BDENF), in addition to Google Scholar, in order to detect articles not indexed in the English and Portuguese languages, published in the last 5 years and available in full. Results: The sample included 6 articles published between 2018 and 2022, from Canada, Australia, Brazil and Korea. The main challenges encountered were the lack of exclusive human resources (doctors and nurses) and the lack of training of ward professionals. Furthermore, the challenge of establishing an assertive trigger to activate the team is discussed, in addition to the need and financing for software that generates automatic alerts based on the insertion of clinical data. Conclusion: It is concluded that regardless of monitoring technologies, the relevance of checking vital signs in a safe, periodic manner, with immediate records, prevails. Furthermore, the need for a work process focused on the use of scores that direct team actions in an assertive manner was highlighted. This study reinforced the importance of periodic training of teams, and also of teams in general, about the purpose of the team, in order to promote communication and coordination between teams in favor of patients.

Five years after the start of RRT in the Institution, study results showed that nurses positively evaluated the program, and 42% of them reported that RRT should also be activated when the unit professional is concerned about any changes in the patient, regardless of stable vital signs (7) . Another investigation identified a similar finding in which 57 (38%) of the 150 yellow code triggers investigated were due to concern for the patient's general condition, with no changes in vital signs (8) .…”

Objective: to analyze the characteristics of the activation of the yellow code in wards and identify the factors associated with adverse events after the Rapid Response Team. Methods: a cross-sectional study with retrospective analysis of medical records of adults admitted to medical or surgical clinic wards of the University Hospital of São Paulo. Results: among the 91 patients, the most frequent signs of triggers (n=107) were peripheral oxygen saturation of less than 90% (40.2%) and hypotension (30.8%). Regarding the associated factors the research identified each minute of attendance of the Rapid Response Team in the wards increased by 1.2% odds of adverse events (twenty-four unplanned admission in the ICU and one cardiac arrest) in the sample (p=0.014). Conclusions: decreased oxygen saturation and hypotension were the main reasons for the triggering, and the length of care was associated with the frequency of adverse events.
