Nurse-Managed Health Centers And Patient-Centered Medical Homes Could Mitigate Expected Primary Care Physician Shortage

Abstract: Numerous forecasts have predicted shortages of primary care providers, particularly in light of an expected increase in patient demand resulting from the Affordable Care Act. Yet these forecasts could be inaccurate because they generally do not allow for changes in the way primary care is delivered. We analyzed the impact of two emerging models of care-the patient-centered medical home and the nursemanaged health center-both of which use a provider mix that is richer in nurse practitioners and physician assist… Show more

“…New workforce models, 22 stratification of the patient population in primary care, 23 putting similar patients together in place or time (or co-location), 23,24 and the power of health IT tools to support work models and care delegation 25 have been identified as ways to improve care value. Most notably, Michael Porter and coauthors proposed a primary care strategy that emphasized the need to stratify a heterogeneous population into subgroups having different needs, 23 with that stratification serving as a foundation for differing work models and metrics.…”

“…NPs are key providers in 1,202 federally funded community health centers and 250 nurse-managed health clinics that embrace team-based care to serve >22 million minority and low-income patients (Hansen-Turton, Bailey, Torres, & Ritter, 2010;National Association of Community Health Centers, 2014). A recent analysis suggested that increasing nurse-managed health centers and expanding patient panel size in patient-centered medical homes could substantially mitigate primary care provider shortages (Auerbach et al, 2013).…”
