NP-59 test for preoperative localization of primary hyperaldosteronism

Abstract: Nuclear medicine tests represent a useful tool in the preoperative localisation of primary hyperaldosteronism with a high sensitivity and positive predictive value. In patients with contraindications to adrenal venous sampling like contrast allergies, or when it is inconclusive, scintigraphy can represent a useful and non-invasive alternative.

“…Adrenocortical scintigraphy for PA (131I-6-betaiodomethyl-19-norcholesterol [NP-59]) is performed under dexamethasone suppression. Compared with conventional planner images, NP-59 SPECT/CT improves both the sensitivity and PPV of subtype testing (sensitivity, 40.9% vs. 81.8%; specificity, 66.7% vs. 66.7%; PPV, 75.0% vs. 85.7%) [95,96]. Therefore, dexamethasonesuppressed adrenocortical scintigraphy is an alternative to AVS in patients with a desire and indication for surgery but who cannot undergo AVS due to iodine allergy or other reasons, in patients who do not wish to undergo AVS, or in patients with inconclusive AVS results.…”

“…et al series [28]. However, the diagnostic accuracy increased in the last available series, reaching a diagnostic accuracy of 83.3% [19], a sensitivity of 90.9%, and a positive predictive value of 83.3%, according to other authors [29]. This improvement is probably related to the introduction of the simultaneous use of integrated SPECT/CT to provide both functional and anatomical information [12].…”

“…The essence of the method is that for the study, one can use a radioactively noticed predecessor of hormones of the adrenal cortex hormones – cholesterol. For this purpose, 131J-methylnorcholesterol (NP-59) was selected [14, 20].…”
