Anti‐centrosome antibodies: Prevalence and disease association in Chinese population

Anti‐centrosome antibodies are rare findings with undefined clinical significance in clinical research. We aimed at investigating the prevalence and clinical significance of anti‐centrosome antibodies in Chinese population. Testing results of total of 281,230 ANA‐positive sera were retrospectively obtained from West China Hospital Sichuan University in China between 2008 and 2017. We retrospectively collected and analysed the clinical and laboratory data of the patients with positive anti‐centrosome antibody. Of the 356 453 patients tested, 281 230 patients had positive antinuclear antibodies (ANAs, 78.9%), but only 78 patients with positive anti‐centrosome antibodies (0.022%), of which 74.4% are females. Diagnoses were established in 69 of 78 patients: 37 cases were autoimmune diseases, mainly including undifferentiated connective tissue diseases (UCTD, 9/37), rheumatoid arthritis (RA, 6/37), Sjögren's syndrome (SS, 5/37) and primary biliary cirrhosis (PBC, 5/37), and the remaining were other autoimmune conditions. The most frequent clinical symptoms of the anti–centrosome‐positive patients were arthralgia and eyes and mouth drying. Additionally, 86.7% of anti‐centrosome antibodies were not associated with other ANA profiles; however, when associated, the most frequent ANA was anti‐U1RNP. Anti‐centrosome antibodies are featured by a low prevalence and female gender predominance. They are correlated with some specific diseases, both autoimmune diseases, especially UCTD, RA, SS and PBC, and non‐autoimmune diseases, such as infection and cancer, which suggests that they might be potential supporting serological markers of these diseases.

Синдром Чарга – Стросса, или эозинофильный гранулематоз с полиангиитом (ЭГПА), впервые описан Дж. Чаргом (Jacob Churg) и Л. Строссом (Lotte Strauss) в 1951 году. ЭГПА представляет собой системный некротизирующий васкулит малых и сред- них сосудов с развитием экстраваскулярных гранулем (обычно с большим количе- ством эозинофилов), эозинофилии и эозинофильной инфильтрации тканей. Воз- никает при астме, начавшейся у взрослых, у пациентов с аллергическим ринитом, назальным полипозом или их сочетанием. Согласно рекомендациям, принятым в 2012 году на консенсусной конференции в Чапел Хилл, ЭГПА определяется как эозинофильный воспалительный некротический гранулематоз с вовлечением дыха- тельных путей и некротическим васкулитом мелких и средних сосудов в сочетании с астмой и эозинофилией. Jacob Churg and Lotte Strauss first described Churg-Strauss syndrome or eosinophilic granulomatosis with polyangiitis (EGPA) in 1951. EGPA is a systemic necrotizing vasculitis of small and medium vessels with the development of extravascular granulomas (usually with a large number of eosinophils), eosinophilia and eosinophilic tissue infiltration. It occurs in asthma that began in adults, in patients with allergic renitis, nasal polyposis or a combination of both. According to the recommendations adopted in 2012 at the Chapel Hill consensus conference, EGPA is defined as eosinophilic inflammatory necrotizing granulomatosis with respiratory involvement and necrotizing vasculitis of small and medium vessels in combination with asthma and eosinophilia.

Eosinophilic granulomatosis with polyangiitis (EGPA), previously called Churg—Strauss syndrome, represents a rare form of ANCA-associated necrotising vasculitis which affects small vessels. This disease is characterized by typical combination of immunological disturbances, hypereosinophilia, severe bronchial asthma, transient pulmonary infiltrates, and kidney injury which is less frequent than in other forms of necrotising vasculitis.Verification of the diagnosis is often hampered by comorbidities, such as chronic obstructive pulmonary disease (COPD) in the patients with long-term smoking history and occupational hazards.In this article, we report a clinical case of EGPA in elderly patient with preceding COPD which caused diagnostic difficulties for this eosinophilic syndrome. Clinical pattern at the beginning of disease was presented by moderate inspiratory dyspnea and cough with small amounts of mucus sputum, which appeared after longterm exposure to chlorine-containing substances. COPD diagnosis in this patient was based on clinical pattern, long smoking experience, and occupational hazards. However, persistence of the symptoms during the ongoing therapy, as well as multidirectional dynamics of transient pulmonary infiltrates found on repeated CT-scans, prompted us to intensify diagnostic search for a systemic disease. Clinical, laboratory and instrumental signs of bronchial asthma were revealed, as well as hypereosinophilia and sensory polyneuropathy, which, if combined with CT-scan data, allowed us to prove the EGPA diagnosis.This case shows that, despite great value of immune diagnostics, with negative blood tests for ANCA, it is necessary to detect mutually complicating comorbid pathology. EGPA was considered the basic diagnosis, and COPD as accompanying disorder, taking into account such reasons as an unfavorable prognosis for EGPA and the need for long-term chemotherapy with systemic corticosteroids and monoclonal antibodies. ANCA-negative testing in the patient, absence of severe kidney and skin lesions allows to suggest better clinical prognosis in this patient.
