Novel dermoscopic features of pityriasis versicolor–like macules in epidermodysplasia verruciformis

Abstract: Epidermodysplasia verruciformis (EV) manifests early in childhood as pityriasis versicolor (PV)-like macules on sun-exposed sites such as the face. These hypopigmented lesions closely resemble commoner pediatric dermatoses such as PV or pityriasis alba. In this report of two cases, we describe the distinguishing dermoscopic features of PV-like macules in EV. Unfocused dotted vessels in a hypopigmented or erythematous background with whitish scales and pigment diluted vellus hairs on dermoscopy should raise the… Show more

“…In addition, it is questioned whether chronic HPV infection may interfere with melanogenesis, thus explaining the hypochromic lesions and dilution of hair pigment. 9 These findings also help to differentiate keratinocytic neoplasms that occur in individuals with epidermodysplasia verruciform.…”

“… 8 Subsequently, other described findings included unfocused dotted vessels with a regular distribution and dilution of vellus hair pigment. 9 Vessel proliferation, seen in other types of warts, is characteristic of HPV virus infection. In addition, it is questioned whether chronic HPV infection may interfere with melanogenesis, thus explaining the hypochromic lesions and dilution of hair pigment.…”

“…There are very few reports of EDV, wherein dermoscopic features have been described. [ 1 2 3 ] Dermoscopy of lesions in EDV has shown demarcated hypopigmented to erythematous background with scaling, unfocused dotted vessels, and diluted hair pigment. SK-like lesions have demonstrated comedo-like openings with abrupt border.…”

“…Unfocues dotted vessels wileyonlinelibrary.com/journal/ped4 in a hypopigmented to erythematous background with pigment diluted vellus hairs on dermoscopy should rase the suspicion of EV in pediatric patients presenting with PV-like lesions. 4 Topical imiquimod which has been shown to stimulate cytotoxic T cell and B cell response is effective in treatment of EV with HPV infections. In our case, after treatment with topical 5% imiquimod cream for three months, the lesions regressed completely and had not recurred at 12-month follow-up (Figure 1D).…”
