Novel Aspects of CPAP Treatment and Interventions to Improve CPAP Adherence

Abstract: Continuous positive airway pressure (CPAP) is an effective treatment for obstructive sleep apnea. However, the success of this treatment is hampered by nonadherence in half of the treated patients. Moreover, in clinical trials, poor adherence reduces adequate exposure required to determine its true effect. There is growing evidence that behavioral interventions, in addition to education, are a promising approach to improving adherence. Behavioral interventions include the use of cognitive behavioral therapy an… Show more

“…These are the first data from a large prospective cohort of patients treated by CPAP, confirming a significant improvement in CPAP adherence during the COVID-19 lockdown. CPAP efficacy is often limited by nonadherence related to a lack of disease-related risk perception [2]. In our case, massive communication about COVID-19, described as a threat to the upper and lower airways, might have motivated patients to be more adherent so as to protect their airways and themselves from the disease, particularly poor adherents.…”

“…Furthermore, a recent review has highlighted the importance of having a partner involved as a member of the team, as long as a positive relationship exists. This can contribute to producing positive adherence outcomes [ 30 ]. While our study focused on the spouse, it is important to note that the influence of a person who can relate to the patient can extend beyond the spouse.…”

“…Continuous positive airway pressure (CPAP) therapy is the most effective treatment for OSA [ 6 , 7 ]. CPAP therapy has been found to improve cognitive processing and daily functioning; reduce daytime sleepiness [ 8 ]; and reduce health care costs, physician visits, and work absences [ 5 ]. Despite the reported benefits of CPAP therapy in treating OSA, its effectiveness is reduced by less-than-optimal adherence/usage.…”

“…Up to 50% of patients who accept CPAP therapy fail to adhere to it [ 8 ]. Patients’ failure to adhere to CPAP therapy is a major factor limiting its potential benefits [ 6 , 9 ].…”
