Herbal and chemical products are used for oral care and biofilm treatment and also have been reported to be controversial in the massive trials conducted in this regard. The present review is aimed at evaluating the potential of relevant herbal and chemical products and comparing their outcomes to conventional oral care products and summarizing the current state of evidence of the antibiofilm properties of different products by evaluating studies from the past eleven years. Chlorhexidine gluconate (CHX), essential oils (EOs), and acetylpyridinium chloride were, respectively, the most commonly studied agents in the included studies. As confirmed by all systematic reviews, CHX and EO significantly control the plaque formation and gingival indices. Fluoride is another interesting reagent in oral care products that has shown promising results of oral health improvement, but the evidence quality needs to be refined. The synergy between natural plants and chemical products should be targeted in the future to accede to the formation of new, efficient, and healthy anticaries strategies. Moreover, to discover their biofilm-interfering or biofilm-inhibiting activities, effective clinical trials are needed. In this review article, therapeutic applications of herbal/chemical materials in oral biofilm infections are discussed in recent years (2010-2022).

Background: Dental plaque is a microbial biofilm that can source various oral diseases. Objectives: This study aimed to investigate the synergistic anti-biofilm effect of chlorhexidine (CHX) and propolis aqueous extract on Streptococcus mutans and Staphylococcus aureus. Methods: This study, performed on October 2021, determined the minimum biofilm inhibitory concentration (MBIC) and minimum biofilm eradication concentration (MBEC) of CHX 0.2% and propolis aqueous extract against S. mutans and S. aureus prepared from the cell bank of the Pasteur Institute of Iran. Also, the in vitro synergistic effect of these two compounds was evaluated on the biofilm inhibition and eradication of the studied bacteria using the checkerboard method. Results: The MBIC of chlorhexidine was 125 μg/mL for both S. aureus and S. mutans, while the MBIC of propolis aqueous extract was 937.5 and 3750 μg/mL, respectively. Also, the MBEC of chlorhexidine was 250 and ≥ 500 μg/mL for S. aureus and S. mutans, while the MBEC of propolis was 1875 and ≥ 7500 μg/mL, respectively. Combining the two substances showed a synergistic effect in inhibiting biofilm formation (P ≤ 0.05) but no significant difference in biofilm eradication. Conclusions: This study concluded that although CHX is one of the most common and vital oral disinfectants in oral diseases, propolis may also be used, as an alternative mouthwash with reliable natural antimicrobial properties, alone or combined with CHX as a moderator of its side effects.

La caries dental es una de las enfermedades más frecuentes a nivel mundial y debido a su prevalecía se comenzó a utilizar la fitoterapia como tratamiento alternativo, la Hibiscus sabdariffa L. posee gran variedad de propiedades terapéuticas de las cuales en este estudio destaca su actividad antibacterial. Objetivo: El objetivo de esta investigación fue determinar el efecto antibacterial in vitro del extracto etanólico de Hibiscus sabdariffa L. sobre cepas de Streptococcus mutans a las 24 y 48 horas. Materiales y métodos: La investigación fue aplicativa de diseño experimental y medición prospectiva. La muestra fue obtenida de pacientes que firmaron el consentimiento informado para así poder obtener las cepas de Streptococcus mutans. Se cultivaron en 16 placas Petri y en cada placa se distribuyeron 7 discos de papel filtro N°4, la técnica fue de difusión por disco de Kirby Bauer con pozos, con un total de 112 discos y pozos; las placas Petri se dividieron en 4 grupos de acuerdo a las concentraciones del extracto etanólico de Hibiscus sabdariffa L. que se obtuvo por maceración con alcohol al 96% para obtener una muestra al 100% extracto etanólico de Hibiscus sabdariffa L. y para obtener las concentraciones de 25%, 50% y 75% se utilizó alcohol para realizar las diluciones. Se consideró la clorhexidina al 0.12% como control positivo y al agua destilada como control negativo. Resultados: Sé demostró que la Hibiscus sabdariffa L. tiene efecto inhibitorio. El mejor efecto se registró a las 48 horas al 100% de concentración. Obteniendo un halo de inhibición de 14.23mm. Conclusión: El extracto etanólico de la Hibiscus sabdariffa L. sí tiene efecto antibacterial in vitro sobre las cepas de Streptococcus mutans.
