Methylphenidate is the most common treatment for attention deficit hyperactivity disorder (ADHD) Attention deficit hyperactivity disorder (ADHD) is the most common pediatric psychiatric disorder (Swanson et al. 1995), with an estimated childhood prevalence of 9% in boys and 3% in girls (Szatmari et al. 1989). Surface symptoms of inattentiveness and impulsivity (Shue and Douglas 1992;Shaywitz and Shaywitz 1988) are often, though not invariably, accompanied by hyperactivity (Gittelman and Mannuzza 1985). ADHD is highly heterogeneous with several distinct subtypes and often comorbid with learning, anxiety, and conduct problems. During childhood, ADHD is associated with greater risk of low academic achievement, grade failure, delinquency (Barkley 1997) and during adulthood, with psychiatric disorders and substance abuse (Wender 1987). Stimulant medication, primarily methylphenidate, is the most common treatment for ADHD. Improved social skills and school performance have also been documented (e.g., Swanson et al. 1995). However, the optimal dosages have not yet been established and clinical anecdotal descriptions report some children as becoming overfocused, cognitively constricted, introverted, and "zombie-like" on higher doses (Swanson et al. 1991; Thurston et al. 1979). These descriptions are not consistently substantiated by empirical studies, however, which show both linear improvements with dose (Douglas et al. 1995;Solanto and Wender 1989) and adverse effects of high dose levels (Dyme et al. 1982;Malone et al. 1988;Sprague and Sleator 1977). “…Parents completed two rating scales: (1) the DBD rating scale (Oosterlaan, Scheres, Antrop, Roeyers, & Sergeant, 2000;Pelham, Gnagy, Greenslade, & Milich, 1992), a symptom severity rating scale containing four scales with the behavioral descriptors of AD/HD inattentive subtype, AD/HD hyperactiveimpulsive subtype, ODD as well as CD according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV; American Psychiatric Association [APA], 1994) and (2) the Child Behavior Checklist (CBCL; Achenbach, 1991;Verhulst, Van der Ende, & Koot, 1996a). This study was aimed at (a) replicating findings of slow and variable response execution and slow response inhibition in Attention Deficit\Hyperactivity Disorder (AD\HD), (b) investigating whether these deficits are specifically related to AD\HD or may also be observed in Oppositional Defiant Disorder (ODD), and children comorbid for AD\HDjODD, and (c) examining the role of activation level in task performance of children with AD\HD. To meet these aims, the stop paradigm was administered at three levels of activation, using a slow, medium, and fast presentation rate of stimuli, to 4 groups of children : 24 AD\HD children, 21 children with ODD, 27 children with comorbid AD\HDjODD, and 41 normal controls. As hypothesized, children with AD\HD exhibited a slow response execution process with considerable variability in the speed of responding compared to normal controls. Slow response execution was also observed in the comorbid AD\HDjODD group but not in the pure ODD group. Larger variability in the speed of responding was common to all disruptive groups compared with controls. In contrast to our hypothesis, no group differences emerged for inhibitory functioning. Finally, the slow event rate condition caused a further deterioration in the speed of the response execution process in both the AD\HD group and ODD group.
